机器人辅助与传统人工全膝关节置换术的临床疗效比较研究OA
A comparative study on the clinical efficacy of robot-assisted versus conventional manual total knee arthroplasty
目的 探讨机器人辅助全膝关节置换术(robotic-assisted total knee arthroplasty,RATKA)与传统全膝关节置换术(total knee arthroplasty,TKA)治疗终末期膝关节骨关节炎(knee osteoarthritis,KOA)的临床疗效对比.方法 回顾性分析2023年1月至2024年12月武汉中西医结合骨科医院骨科收治的56例终末期KOA患者,按手术方式分为RATKA组(27例)和传统TKA组(29例).记录两组围手术期指标(手术时间、截骨时间、术中出血量、术后引流量、血红蛋白差值),对比术前及术后髋-膝-踝角(hip-knee-ankle,HKA)偏移、冠状面股骨组件角(coronal femoral component angle,CFCA)、冠状面胫骨组件角(coronal tibial component angle,CTCA)、胫骨平台后倾角(posterior tibial slope angle,PTSA)等影像学指标,评估术前及术后末次随访时视觉模拟评分(visual analogue score,VAS)、膝关节活动度(range of motion,ROM)、膝关节协会评分(knee society score,KSS)等临床疗效指标,并统计并发症发生情况.结果 所有患者手术均顺利完成,并获得6~28个月随访,平均(14.04±5.82)个月.两组患者术后并发症发生率、术中出血量、截骨时间、术后引流量比较,差异无统计学意义(P>0.05),RATKA组的手术时间长于传统TKA组,Hb差值低于传统TKA组(P<0.05).两组患者术前HKA偏移、CFCA、CTCA、PTSA比较,差异无统计学意义(P>0.05);两组患者末次随访HKA偏移较术前明显降低,且RATKA组更低于传统TKA组(P<0.05);两组患者末次随访CFCA与术前相比及组间比较,差异无统计学意义(P>0.05);两组患者末次随访CTCA与术前相比,差异有统计学意义(P<0.05),但组间比较,差异无统计学意义(P>0.05);两组患者末次随访PTSA比较,RATKA组高于传统TKA组(P<0.05).两组患者末次随访VAS评分、ROM、KSS评分均较术前有改善,差异有统计学意义(P<0.05),但两组间比较,差异无统计学意义(P>0.05).结论 国产HURWA机器人辅助TKA与传统TKA相比,围术期出血量少,可显著提高下肢对线及假体放置精准度.
Objective To compare the clinical efficacy of robotic-assisted total knee arthroplasty(RATKA)versus conventional total knee arthroplasty(TKA)in the treatment of end-stage knee osteoarthritis(KOA).Methods A retrospective analysis was conducted on 56 patients with end-stage KOA between January 2023 and December 2024 at Wuhan Hospital of Integrated Traditional Chinese and Western Medicine Orthopedics.Among them,27 patients underwent RATKA(RATKA group),and 29 underwent conventional TKA(conventional TKA group).Perioperative indicators,including operative time,osteotomy time,intraoperative blood loss,postoperative drainage volume,and hemoglobin difference,were recorded.Radiographic parameters such as hip-knee-ankle angle(HKA)deviation,coronal femoral component angle(CFCA),coronal tibial component angle(CTCA),and posterior tibial slope angle(PTSA)inclination were compared between the two groups preoperatively and at the last postoperative follow-up.Clinical efficacy indicators,including the visual analogue score(VAS),range of motion(ROM),and knee society score(KSS),were evaluated preoperatively and at the final follow-up.Complications during the perioperative and follow-up periods were also recorded.Results All surgeries were successfully completed,with a follow-up period ranging from 6 to 28 months(mean:14.04±5.82 months).No significant differences were observed in the incidence of postoperative complications,intraoperative blood loss,osteotomy time,or postoperative drainage volume between the two groups(P>0.05).However,the operative time was longer in the RATKA group,while the hemoglobin difference was lower compared to the conventional TKA group(P<0.05).Preoperative HKA deviation,CFCA,CTCA,and PTSA showed no significant differences between the two groups(P>0.05).At the final follow-up,HKA deviation significantly decreased in both groups compared to preoperative values,with a greater reduction in the RATKA group(P<0.05).No significant differences were found in CFCA between the two groups postoperatively or compared to preoperative values(P>0.05).Although CTCA showed significant changes postoperatively within each group(P<0.05),no significant differences were observed between the groups(P>0.05).The PTSA was higher in the RATKA group than in the conventional TKA group at the final follow-up(P<0.05).Both groups exhibited significant improvements in VAS score,ROM,and KSS score postoperatively(P<0.05),but no significant differences were found between the groups(P>0.05).Conclusion Compared with traditional total knee arthroplasty(TKA),domestic HURWA robot-assisted TKA is associated with less perioperative blood loss,and can significantly improve the accuracy of lower limb alignment and prosthesis implantation.
