Chronic diarrhea is characterized by a complex pathogenesis,a protracted course,and a tendency to persist without resolution.It often involves multiple organs,including the lung,spleen,liver,and kidney,and is accompanied by intertwined pathological factors such as phlegm,dampness,blood stasis,and stagnation,leading to obstruction of the intestinal tract and dysfunction of transmission,thereby rendering the condition refractory.Clinically,it commonly presents with complicated syndromes marked by Zang-fu disharmony,a mixture of cold and heat,and the coexistence of deficiency and excess,for which conventional therapeutic approaches often achieve limited efficacy.Professor SHEN Hong advocates that,in response to the changing patterns of chronic diarrhea,treatment should adopt"modified strategies",emphasizing accurate identification of pathogenesis and the establishment of treatment principles based on syndrome differentiation to achieve precise regulation.This paper systematically expounds his six therapeutic methods for the differentiation and treatment of chronic diarrhea.These methods closely follow the evolution of pathogenesis and skillfully integrate the harmonization of Zang-fu organs,the coordination of cold and warm approaches,and the balanced application of purgation and astringency,fully reflecting the essence of flexible and dynamic syndrome differentiation and treatment in traditional Chinese medicine.
