目的 分析癫痫患儿照顾者习得性无助感的现状,并基于结构方程模型探讨其影响因素的路径,为干预措施的制定与实施提供参考.方法 2024年1月至2025年6月,采用便利抽样法选取至莆田市儿童医院接受治疗的222例癫痫患儿及其照顾者作为研究对象.采用一般资料调查表、习得性无助量表(LHS)、社会支持评定量表(SSRS)、疾病不确定感家属量表(MUIS-FM)及Zarit照顾者负担量表(ZBI)对患儿照顾者进行调查.结果 纳入的222名癫痫患儿照顾者的LHS、SSRS、MUIS-FM、ZBI得分依次为(55.38±5.54)分、(35.76±4.89)分、(89.29±9.06)分、(38.91±5.38)分.相关性分析结果显示,癫痫患儿照顾者LHS得分与MUIS-FM、ZBI得分呈正相关,与SSRS得分呈负相关(均P<0.05).多元线性回归分析显示,ZBI、SSRS及MUIS-FM得分均为癫痫患儿照顾者习得性无助感的独立影响因素(均P<0.05).结构方程模型显示,社会支持可对习得性无助感产生直接效应(占总效应的49.36%),并通过疾病不确定感和照顾负担对习得性无助感产生间接效应(占总效应的50.64%).结论 癫痫患儿照顾者的习得性无助感处于中等水平.社会支持不仅直接影响习得性无助感,还通过疾病不确定感与照顾负担的链式中介路径产生间接影响.提示医护人员应关注照顾者的社会支持状况,通过提升其社会支持水平、降低疾病不确定感与照顾负担,从而缓解其无助感,改善心理健康状况及照护质量.

Objective To analyze the current status of learned helplessness in caregivers of children with epilepsy and explore the pathways of its influencing factors based on a structural equation model,aiming to provide a reference for the development and implementation of interventions.Methods From January 2024 to June 2025,222 caregivers of children with epilepsy treated at Putian Children's Hospital were selected by convenience sampling.They were investigated using the General Information Questionnaire,Learned Helplessness Scale(LHS),Social Support Rating Scale(SSRS),Managing Uncertainty in Illness Scale-Family Member(MUIS-FM)and Zarit Burden Interview(ZBI).Results The LHS,SSRS,MUIS-FM and ZBI scores of the 222 caregivers of children with epilepsy were(55.38±5.54)points,(35.76±4.89)points,(89.29±9.06)points and(38.91±5.38)points,respectively.Correlation analysis showed that LHS scores were positively correlated with MUIS-FM and ZBI scores while negatively correlated with SSRS scores in caregivers of children with epilepsy(P<0.05 for all).Multivariate linear regression analysis revealed that ZBI,SSRS and MUIS-FM scores were independent factors influencing learned helplessness in caregivers of children with epilepsy(P<0.05 for all).The structural equation model indicated that social support had a direct effect on learned helplessness,accounting for 49.36%of the total effect,and an indirect effect through uncertainty in illness and caregiver burden,accounting for 50.64%of the total effect.Conclusion The learned helplessness in caregivers of children with epilepsy is at a moderate level.Social support not only directly influences learned helplessness but also exerts an indirect effect through the chain mediation of uncertainty in illness and caregiver burden.Healthcare professionals should pay attention to the social support status of caregivers.By enhancing caregivers'social support levels and reducing uncertainty in illness and caregiver burden,healthcare professionals can relieve their helplessness,improve mental health and enhance caregiving quality.
