健脾补肾方联合吉西他滨+顺铂化疗方案治疗三阴性乳腺癌临床研究OA
Clinical study of Jianpi Bushen decoction combined with gemcitabine plus cisplatin chemotherapy regimen in the treatment of triple negative breast cancer
目的:探究健脾补肾方联合吉西他滨+顺铂(GP)化疗方案治疗三阴性乳腺癌(TNBC)临床疗效.方法:选取TNBC、辨证为脾肾亏虚证患者94例进行研究,经简单随机化(随机数表法)分为对照组和观察组各47例,分别给予GP化疗方案、健脾补肾方联合GP化疗方案治疗.比较两组中医疗效、近期实体瘤疗效、治疗前后中医症状评分、血清学指标[胸苷激酶1(TK1)、血小板/淋巴细胞值(PLR)和中性粒细胞/淋巴细胞值(NLR)]变化,记录治疗期间不良反应.结果:观察组临床有效率93.62％、临床缓解率70.21％高于对照组的78.72％、46.81％(P＜0.05).治疗结束后,两组主症(多食欲呕、胸脘痞闷、四肢不温、脱发)、次症(面色㿠白、神疲乏力、腹痛腹胀腹鸣)评分均明显降低(P＜0.05),且观察组更低(P＜0.05).治疗结束后,两组TK1、PLR和NLR表达量均明显降低(P＜0.05);且观察组更低(P＜0.05).治疗期间,观察组在恶心呕吐、脱发、贫血、泌尿系统异常和肝功能异常发生率方面低于对照组(P＜0.05).结论:健脾补肾方联合GP化疗方案能显著提高TNBC临床疗效,改善临床症状,调节血清肿瘤因子和炎症因子水平,减轻化疗药物不良反应.
Objective:To explore the clinical efficacy of Jianpi Bushen decoction combined with gemcitabine plus cisplatin(GP)chemotherapy regimen in the treatment of triple negative breast cancer(TNBC).Methods:94 patients with TNBC with spleen-kidney deficiency syndrome were selected for study,and were divided into control group and observation group by simple randomization(random number table method),with 47 cases in each group.GP chemo-therapy regimen and Jianpi Bushen decoction combined with GP chemotherapy regimen were given respectively.The changes of TCM efficacy,short-term solid tumor efficacy,TCM symptoms scores,serological indexes[thymidine ki-nase 1(TK1),platelet to lymphocyte ratio(PLR)and neutrophil to lymphocyte ratio(NLR)]before and after treat-ment were compared between the two groups,and adverse reactions during treatment were recorded.Results:The clinical effective rate and clinical remission rate with 93.62％and 70.21％in observation group were higher than 78.72％and 46.81％in control group(P＜0.05).After the end of treatment,the scores of primary symptoms(ano-rexia and vomiting,chest fullness and stuffiness,cold limbs,alopecia)and secondary symptoms(pale complexion,lassitude,abdominal pain,abdominal distension and abdominal stridor)in the two groups were significantly decreased(P＜0.05),and the scores in observation group were lower(P＜0.05).The expression levels of TK1,PLR and NLR in the two groups were significantly decreased after the end of treatment(P＜0.05),and the levels were lower in ob-servation group(P＜0.05).During treatment,the incidence rates of nausea and vomiting,alopecia,anemia,urinary system abnormalities and liver dysfunction in observation group were lower than those in control group(P＜0.05).Conclusion:Jianpi Bushen decoction combined with GP chemotherapy regimen can significantly enhance the clinical efficacy of TNBC,improve the clinical symptoms,regulate the levels of serum tumor factors and inflammatory fac-tors,and relieve the toxic and side effects of chemotherapy drugs.
