乳腺癌内分泌治疗中医证候研究OA
TCM syndrome study in breast cancer patients undergoing endocrine therapy
目的:运用因子分析和聚类分析探讨乳腺癌内分泌治疗患者的中医证候分布规律.方法:采用横断面的研究设计,收集2025年1月至2025年12月北京中医药大学第三附属医院、厦门市中医院及重庆市中医院乳腺癌内分泌治疗患者的一般资料及中医四诊信息,录入Excel 2019进行频数分析,应用SPSS 27.0进行因子分析和聚类分析,对患者中医证候特点进行归纳总结.结果:共纳入480例乳腺癌内分泌治疗患者,保留频率≥10％的中医证候条目54项,睡眠不实、失眠、潮热、自汗、口渴、脉弦、脉细、健忘、舌黯红、酸重痛等为最常见的症状或体征.通过因子分析得出21个公因子,进一步分析提示病性证素涉及12项,以阴虚、湿、阳虚最常见;病位证素包括9项,主要集中在肾、肝、脾.基于21个公因子的聚类分析结果显示,常见证型包括肝肾亏虚证(42.08％)、脾虚湿蕴证(26.25％)、肝郁化火证(16.04％)、气阴两虚证(15.63％).结论:乳腺癌内分泌治疗患者病位主要在肾、肝、脾,中医证型多为肝肾亏虚证、脾虚湿蕴证、肝郁化火证和气阴两虚证.
Objective:To investigate the distribution patterns of traditional Chinese medicine(TCM)syndromes in patients with breast cancer undergoing endocrine therapy using factor analysis and cluster analysis.Methods:A cross-sectional study design was adopted.General data and TCM four-diagnosis information were collected from pa-tients receiving endocrine therapy for breast cancer at the Third Affiliated Hospital of Beijing University of Chinese Medicine,Xiamen Hospital of Traditional Chinese Medicine,and Chongqing Hospital of Traditional Chinese Medicine between January 2025 and December 2025.Data were entered into Excel 2019 for frequency analysis,and factor anal-ysis and cluster analysis were performed using SPSS 27.0 to summarize the TCM syndrome characteristics of the pa-tients.Results:A total of 480 patients with breast cancer on endocrine therapy were enrolled.Fifty-four TCM syn-drome items with a frequency ≥10％were retained.The most common symptoms or signs included restless sleep,insomnia,hot flashes,spontaneous sweating,thirst,wiry pulse,thready pulse,forgetfulness,dark red tongue,and soreness and heaviness pain.Factor analysis extracted 21 common factors,which further revealed 12 disease nature syndrome elements,with Yin deficiency,dampness,and Yang deficiency being the most prevalent;and 9 disease loca-tion syndrome elements,mainly involving the kidney,liver,and spleen.Cluster analysis based on the 21 common fac-tors showed that the common syndrome types included liver-kidney deficiency syndrome(42.08％),spleen deficiency with dampness accumulation syndrome(26.25％),liver depression transforming into fire syndrome(16.04％),and Qi-Yin defi-ciency syndrome(15.63％).Conclusion:The disease location in breast cancer patients undergoing endocrine therapy is mainly in the kidney,liver,and spleen,and the TCM syndrome types are predominantly liver-kidney deficiency syndrome,spleen deficiency with dampness accumulation syndrome,liver depression transforming into fire syndrome,and Qi-Yin deficiency syndrome.
