益肾固胎颗粒通过调控Th17/Treg细胞平衡改善复发性流产的作用机制研究OA

目的:探讨益肾固胎颗粒通过调节Th17/Treg细胞平衡改善复发性流产(RSA)小鼠妊娠结局的作用机制.方法:将30只CBA/J雌鼠随机分为对照组、正常妊娠组、RSA模型组、益肾固胎颗粒组及环孢素A组.妊娠第0~14天,益肾固胎颗粒组、环孢素A组分别按3.6 g/kg、25 mg/kg剂量灌胃.末次给药后,经HE染色观察胎盘病理;ELISA法检测血清白细胞介素-6(IL-6)、白细胞介素-17α(IL-17α)、转化生长因子-β(TGF-β)以及白细胞介素-10(IL-10)水平;流式细胞术分析外周血Th17/Treg的细胞比例.结果:与RSA模型组比较,益肾固胎颗粒组胎盘组织病理损伤显著改善,炎性细胞浸润及组织肿胀明显减轻;血清IL-6、IL-17α水平分别降低42.7%和56.9%(P<0.01),TGF-β 和 IL-10 表达量提升 2.1 倍和 1.7 倍;Th17 细胞比例由(7.5±0.8)%降至(3.2±0.5)%,Treg细胞比例由(8.2±0.9)%升至(12.7±1.1)%,Th17/Treg比值由0.32恢复至0.25(P<0.01).结论:益肾固胎颗粒可能通过调控Th17/Treg细胞平衡,增强母-胎界面免疫耐受,从而改善RSA妊娠结局,为中医"补肾安胎"法提供了分子免疫学依据.

Objective:To investigate the mechanism by which Yishen Gutai granules improve pregnancy out-comes in mice with recurrent spontaneous abortion(RSA)by regulating the Th17/Treg cell balance.Methods:Thirty CBA/J female mice were randomly divided into five groups:the control group,the normal pregnancy group,the RSA model group,the Yishen Gutai granules group,and the cyclosporine A group.From day 0 to day 14 of gestation,mice in the Yishen Gutai granules and cyclosporine A groups were administered 3.6 g/kg and 25 mg/kg,respectively,by gavage.After the final administration,placental pathology was observed via hematoxylin-eosin(HE)staining.Serum levels of interleukin-6(IL-6),interleukin-17α(IL-17α),transforming growth factor-β(TGF-β),and interleukin-10(IL-10)were detected by enzyme-linked immunosorbent assay(ELISA).The proportion of Th17 and Treg cells in peripheral blood was analyzed by flow cytometry.Results:Compared with the RSA model group,placental pathologi-cal injury in the Yishen Gutai granules group was significantly improved,with markedly alleviated inflammatory cell infiltration and tissue swelling.Serum levels of IL-6 and IL-17α decreased by 42.7%and 56.9%,respectively(P<0.01),while expression levels of TGF-β and IL-10 increased by 2.1-fold and 1.7-fold,respectively.The proportion of Th17 cells decreased from(7.5±0.8)%to(3.2±0.5)%,the proportion of Treg cells increased from(8.2±0.9)%to(12.7±1.1)%,and the Th17/Treg ratio was restored from 0.32 to 0.25(P<0.01).Conclusion:Yishen Gutai granules may improve pregnancy outcomes in RSA mice by regulating the Th17/Treg cell balance and enhan-cing immune tolerance at the maternal-fetal interface.These findings provide a molecular immunological basis for the traditional Chinese medicine method of"tonifying the kidney to secure the fetus".
