目的 探讨不同脂耐量非肥胖男性在高脂餐后血清皮质醇水平的变化特点,并分析其影响因素.方法 选取90名非肥胖男性受试者,均接受口服脂肪耐量试验(OFTT),检测OFTT 0、2、4、6 h血清皮质醇、总胆固醇(TC)、甘油三酯(TG)、低密度脂蛋白胆固醇(LDL-C)、高密度脂蛋白胆固醇(HDL-C)、游离脂肪酸(FFA)及空腹血糖、空腹胰岛素(FINS).根据OFTT 0、4 h TG水平,将脂耐量分为脂耐量正常(NFT)、脂耐量减低(IFT)、高甘油三酯血症(HTG).比较不同脂耐量受试者血清皮质醇水平变化,采用单因素及多元线性回归分析血清皮质醇水平的影响因素.结果 90名受试者的脂耐量分别为NFT、IFT、HTG,各30名.同一脂耐量比较:所有受试者高脂餐后血清皮质醇及FFA水平均呈先降后升趋势,皮质醇于OFTT 4 h降至最低(P均<0.05),FFA于OFTT 2 h降至最低(P均<0.05),TG于OFTT 4 h达峰值(P均<0.05).不同脂耐量间比较:餐前,HTG受试者腰围、血清TG、TC、LDL-C、FFA及皮质醇水平均高于NFT、IFT受试者(P均<0.05).餐后各时间点,不同脂耐量受试者血清FFA、TG水平比较差异均有统计学意义(P均<0.05);OFTT 4 h,HTG受试者血清皮质醇水平高于NFT、IFT受试者(P均<0.05);OFTT 6 h,IFT、HTG受试者血清皮质醇水平均高于NFT受试者(P均<0.05).校正混杂因素后,餐前血清FFA、TG水平是同期血清皮质醇水平的独立影响因素(P均<0.05);OFTT 6 h血清FFA水平是同期血清皮质醇水平的独立影响因素(P<0.05).结论 脂耐量异常程度较重的非肥胖男性血清皮质醇水平更高,空腹血清FFA、TG水平及餐后6 h血清FFA水平是血清皮质醇水平的影响因素.

Objective To investigate postprandial changes in serum cortisol levels after a high-fat meal in non-obese men with different degrees of fat tolerance and to analyze the associated factors.Methods A total of 90 male par-ticipants underwent an oral fat tolerance test(OFTT).Based on fasting triglyceride(TG)levels and TG levels at OFTT 4 h,participants were classified into three groups(n=30 each):normal fat tolerance(NFT),impaired fat tolerance(IFT),and hypertriglyceridemia(HTG).All participants received a high-fat meal(1,500 kcal,with 60%of total ener-gy derived from fat).Blood samples were collected at OFTT 0,2,4,and 6 h,and serum cortisol,free fatty acids(FFA),TG,total cholesterol(TC),low-density lipoprotein cholesterol(LDL-C),high-density lipoprotein cholesterol(HDL-C),fasting blood glucose,and fasting insulin(FINS)were measured.Multivariate linear regression analysis was performed to identify factors associated with cortisol levels.Results Within-group comparisons showed that cortisol and FFA levels decreased initially and then increased after the high-fat meal in all three groups.Cortisol levels reached their nadir at OFTT 4 h(all P<0.05),whereas FFA levels reached their lowest point at OFTT 2 h(all P<0.05).TG levels peaked at OFTT 4 h in all groups(all P<0.05).Between-group comparisons showed that,at baseline,waist circumfer-ence,TG,TC,LDL-C,FFA,and cortisol levels were significantly higher in the HTG group than in the NFT and IFT groups(all P<0.05).At all postprandial time points,significant differences were observed in FFA and TG levels among the three groups(all P<0.05).At OFTT 4 h,cortisol levels were significantly higher in the HTG group than in the NFT and IFT groups(all P<0.05).At OFTT 6 h,cortisol levels were significantly higher in the IFT and HTG groups than in the NFT group(all P<0.05).Multivariate linear regression analysis showed that,after adjustment for confounding fac-tors,preprandial FFA and TG levels were independent risk factors for cortisol levels(all P<0.05),whereas the FFA lev-el at OFTT 6 h was an independent risk factor for serum cortisol levels(P<0.05).Conclusion Serum cortisol levels were higher in non-obese men with more severe impairment of fat tolerance.Fasting FFA and TG levels,as well as serum FFA levels at 6 h postprandially,were independent risk factors for serum cortisol levels.
