Effects of electroacupuncture at acupoints on glucose metabolism and endoplasmic reticulum stress in skeletal muscle of type 2 diabetic rats

目的 观察电针穴位刺激对2型糖尿病(T2DM)大鼠糖代谢及骨骼肌内质网应激(ERS)的影响,探讨其改善糖代谢可能的作用机制.方法 按随机数字表法将18只大鼠分为空白组、模型组、电针组,每组6只.空白组给予普通饲料喂养;模型组、电针组给予高糖高脂饲料喂养、腹腔注射1%STZ溶液制备T2DM模型.电针组给予电针干预,每天1次,每周6次,干预6周;空白组、模型组仅予抓取与固定20 min,不予干预.对比各组糖代谢指标空腹血糖(FBG)、血清空腹胰岛素(FINS)、胰岛素抵抗指数(HOMA-IR),原子力显微镜(AFM)观察骨骼肌细胞内质网(ER)超微结构,用Western blotting法检测骨骼肌组织中ERS相关蛋白葡萄糖调节蛋白78(GRP78)、转录激活因子6(ATF6)、蛋白酪氨酸磷酸酶1B(PTP1B)及胰岛素受体底物1(IRS1)蛋白表达.结果 与空白组比较,模型组造模后及干预后FBG、血清FINS、HOMA-IR均高(P均<0.05),ER呈网状分布的管状结构,骨骼肌组织GRP78、ATF6、PTP1B蛋白表达高而IRS1蛋白表达低(P均<0.05).与模型组比较,电针组干预后FBG、血清FINS、HOMA-IR均低(P均<0.05),ER形态趋于正常,骨骼肌组织GRP78、ATF6、PTP1B蛋白表达低而IRS1蛋白表达高(P均<0.05).结论 电针可能通过调节ERS相关蛋白的表达来抑制骨骼肌ERS,上调IRS1蛋白表达,从而改善T2DM大鼠的糖代谢.

Objective To observe the effects of electroacupuncture at acupoints on glucose metabolism and endoplas-mic reticulum stress(ERS)in the skeletal muscle of rats with type 2 diabetes mellitus(T2DM),and to explore the poten-tial mechanism underlying its improvement of glucose metabolism.Methods According to the random number table meth-od,18 rats were divided into the blank group,model group,and electroacupuncture group,with 6 rats in each group.Rats in the blank group were fed with a normal diet.Rats in the model group and electroacupuncture group were fed with a high-fat and high-sugar diet and intraperitoneally injected with 1%streptozotocin(STZ)solution to establish T2DM mod-els.Rats in the electroacupuncture group received electroacupuncture intervention once a day,6 times a week for 6 weeks.Rats in the blank group and model group were only grasped and fixed for 20 minutes without intervention.The glu-cose metabolism indicators,including fasting blood glucose(FBG),serum fasting insulin(FINS),and homeostasis model assessment of insulin resistance(HOMA-IR),were compared among the groups.The ultrastructure of endoplasmic reticu-lum in skeletal muscle cells was observed using atomic force microscopy(AFM).The expression levels of endoplasmic re-ticulum stress-related proteins glucose-regulated protein 78(GRP78),activating transcription factor 6(ATF6),protein ty-rosine phosphatase 1B(PTP1B),and insulin receptor substrate 1(IRS1)in the skeletal muscle tissues were detected by Western blotting.Results Compared with the blank group,the model group showed significantly higher FBG,serum FINS,and HOMA-IR after modeling and intervention(all P<0.05),with a reticular tubular structure of the endoplasmic reticulum,higher protein expression levels of GRP78,ATF6,and PTP1B,and lower IRS1 protein expression in skeletal muscle tissues(all P<0.05).Compared with the model group,the electroacupuncture group showed significantly lower FBG,serum FINS,and HOMA-IR after intervention(all P<0.05),with normalized endoplasmic reticulum morphology,lower protein expression levels of GRP78,ATF6,and PTP1B,and higher IRS1 protein expression in skeletal muscle tis-sue(all P<0.05).Conclusion Electroacupuncture may improve glucose metabolism in T2DM rats by regulating the ex-pression of ERS-related proteins,inhibiting ERS in skeletal muscle,and up-regulating IRS1 protein expression.
