早期糖尿病肾脏病患者葡萄糖目标范围内时间与骨转换标志物的关系OA
Relationship between time in range and bone turnover markers in patients with early diabetic kidney disease
目的 探讨早期糖尿病肾脏病患者葡萄糖目标范围内时间(TIR)与骨转换标志物的关系.方法 选择处于估算肾小球滤过率(eGFR)G1期的2型糖尿病患者190例,依据尿白蛋白/肌酐比值(ACR)分为单纯T2DM患者59例、G1A2患者70例、G1A3患者61例.收集一般资料,包括年龄、性别、BMI及糖尿病病程;实验室检测指标,包括肾功能指标血清肌酐(SCr)、eGFR、ACR;糖代谢指标,包括空腹血糖(FPG)、餐后血糖(PPG)、糖化血红蛋白(HbA1c);骨代谢指标25-羟维生素D[3 25(OH)VitD3]及骨转换标志物Ⅰ型前胶原氨基端前肽(P1NP)、β-Ⅰ型胶原C端交联肽(β-CTX);基于连续葡萄糖监测计算TIR.采用协方差分析比较不同ACR分层患者骨转换标志物差异,采用Pearson相关分析和偏相关分析评估TIR与骨转换标志物的相关性,采用限制性立方样条(RCS)和分位数回归分析TIR与β-CTX的关系.结果 不同ACR分层患者性别构成、糖尿病病程、FPG、PPG、HbA1c、TIR、ACR、SCr、25(OH)VitD3及P1NP比较差异均具有统计学意义(P均<0.05),年龄、BMI、eGFR及β-CTX比较差异均无统计学意义(P均>0.05).校正年龄、性别及BMI后,25(OH)VitD3随ACR升高呈下降趋势(趋势性P<0.001),P1NP随ACR升高呈上升趋势(趋势性P=0.004),β-CTX总体差异无统计学意义,但呈升高趋势(趋势性P=0.066).相关性分析显示,TIR与25(OH)VitD3 呈正相关(r=0.206,P=0.004),与 β-CTX 及 P1NP 呈负相关(r 分别为-0.162、-0.199,P 分别为 0.026、0.006).偏相关分析显示,在控制HbA1c及年龄、性别、BMI等因素后,TIR仍与25(OH)VitD3呈正相关(r=0.198,P=0.007),与P1NP及β-CTX呈负相关(r分别为-0.257、-0.218,P分别为<0.001、0.003).RCS分析显示,TIR与β-CTX存在非线性关联(P=0.043);分位数回归分析显示,在β-CTX第90百分位数处,TIR与β-CTX呈负相关(β=-0.021,P=0.025).结论 早期糖尿病肾脏病患者已出现骨转换异常趋势,TIR与骨转换标志物存在独立关联,该关联在高骨吸收水平人群中更为明显.
Objective To investigate the relationship between time in range(TIR)and bone turnover markers in early diabetic kidney disease(DKD)patients.Methods We enrolled 190 patients with type 2 diabetes mellitus(T2DM)with estimated glomerular filtration rate(eGFR)in stage G1.According to urinary albumin-to-creatinine ratio(ACR),patients were divided into three groups:T2DM without albuminuria(n=59),eGFR G1/ACR A2(G1A2,n=70),and eGFR G1/ACR A3(G1A3,n=61).General data including age,sex,body mass index(BMI),and diabetes du-ration were collected.Laboratory indicators included serum creatinine(SCr),eGFR,ACR,fasting plasma glucose(FPG),postprandial glucose(PPG),glycated hemoglobin(HbA1c),25-hydroxyvitamin D3[25(OH)VitD3],procolla-gen type I N-terminal propeptide(P1NP),and β-isomerized C-terminal telopeptide of type I collagen(β-CTX).TIR was calculated from continuous glucose monitoring(CGM)data.Analysis of covariance(ANCOVA)was used to compare the differences in bone turnover markers among patients stratified by different ACR levels.Pearson correlation analysis and partial correlation analysis were adopted to evaluate the correlations between TIR and bone turnover markers.Restricted cubic spline(RCS)and quantile regression were applied to explore the relationship between TIR and β-CTX.Results Sex distribution,diabetes duration,FPG,PPG,HbA1c,TIR,ACR,SCr,25(OH)VitD₃,and P1NP differed significant-ly among the three groups(all P<0.05),while age,BMI,eGFR,and β-CTX showed no significant differences(all P>0.05).After adjusting for age,sex,and BMI,25(OH)VitD3 showed a decreasing trend with increasing ACR(P for trend<0.001),and P1NP showed an increasing trend(P for trend=0.004).β-CTX showed no significant overall differ-ence among groups,but an upward trend(P for trend=0.066).Pearson correlation analysis showed that TIR was positive-ly correlated with 25(OH)VitD₃(r=0.206,P=0.004),and was negatively correlated with β-CTX(r=-0.162,P=0.026)and P1NP(r=-0.199,P=0.006).Partial correlation analysis,controlling for HbA1c,age,sex,and BMI,showed that TIR remained positively correlated with 25(OH)VitD₃(r=0.198,P=0.007),and negatively correlated with P1NP(r=-0.257,P<0.001)and β-CTX(r=-0.218,P=0.003).RCS analysis revealed a non-linear associa-tion between TIR and β-CTX(P=0.043).Quantile regression analysis showed that TIR was negatively associated with β-CTX at the 90th percentile(β=-0.021,P=0.025).Conclusions Early-stage DKD patients showed a trend of abnormal bone turnover.TIR is independently associated with bone turnover markers,and this association is more pro-nounced in the high bone resorption population.
