血清颗粒酶B与特应性皮炎临床特征的相关性:一项横断面研究OA
Association between serum granzyme B and clinical features of atopic dermatitis:A cross-sectional study
目的 通过检测特应性皮炎(AD)患者血清中颗粒酶B的水平,分析其与疾病严重程度和临床特征的相关性.方法 收集90例AD患者和30例健康对照者血清标本,运用ELISA试验检测血清中颗粒酶B水平,分析其与疾病严重程度、IgE水平、嗜酸性粒细胞计数以及IL-18的相关性.结果 AD患者血清中颗粒酶B水平较健康对照组明显升高(P<0.001),且与SCORAD评分(P<0.001)、嗜酸性粒细胞计数(P<0.05)、IL-18水平(P<0.001)均呈正相关.轻、中、重不同病情严重程度AD患者的颗粒酶B水平差异显著(P<0.001),且两两比较差异均有统计学意义(均P<0.05).内、外源型的AD颗粒酶B水平无差异(P>0.05).AD中每升高1个单位(pg/mL)颗粒酶B平均会引起约14.28个单位的IL-18水平升高(P<0.001),并且颗粒酶B作为AD疾病严重程度的独立危险因素OR值大于嗜酸性粒细胞计数的OR值(P<0.001).Logistic回归分析显示颗粒酶B和嗜酸性粒细胞均为AD疾病严重程度的独立危险因素(P<0.001).颗粒酶B的ROC曲线下面积(AUC)为0.649,小于嗜酸性粒细胞计数的0.826,当取最佳诊断值时两者的灵敏度分别为37.8%和65.6%,特异度均为96.7%.结论 血清颗粒酶B在AD患者血清中显著升高,是疾病严重程度的独立危险因素,且与IL-18及嗜酸性粒细胞水平正相关;其特异性高但灵敏度较低,或许更适合作为辅助评估指标,而非独立筛查工具.
Objective To assess the serum granzyme B levels in patients with atopic dermati-tis,and to analyze their association with disease severity and clinical characteristics.Methods Ninety patients with atopic dermatitis(AD)and 30 healthy controls were enrolled,and their ser-um specimens were collected.The ELISA assay was used to measure serum granzyme B levels,and their correlations with disease severity,IgE levels,eosinophil counts,and IL-18 were ana-lyzed.Results Serum granzyme B levels were significantly elevated in AD patients compared with healthy controls(P<0.001)and positively correlated with SCORAD scores(P<0.001),eosin-ophil counts(P<0.05),and IL-18 levels(P<0.001).Granzyme B levels differed significantly among mild,moderate,and severe AD subgroups(P<0.001),with all pairwise comparisons reaching statistical significance(P<0.05 or P<0.001).No significant difference in granzyme B levels was observed between endogenous and exogenous AD(P>0.05).A unit increase(pg/mL)in granzyme B was associated with an average increase of approximately 14.28 units in IL-18 levels(P<0.001).Moreover,granzyme B was identified as an independent risk factor for AD severity,with an odds ratio(OR)greater than that of eosinophil counts(P<0.001).Logistic re-gression analysis revealed that both granzyme B and eosinophil counts were independent risk fac-tors for AD severity(P<0.001).ROC curve analysis showed that the area under the curve(AUC)for granzyme B was 0.649,lower than the 0.826 for eosinophil counts.At the optimal cutoff values,the sensitivities of granzyme B and eosinophil counts were 37.8%and 65.6%,re-spectively,while both exhibited a specificity of 96.7%.Conclusions Serum granzyme B is sig-nificantly elevated in patients with atopic dermatitis and serves as an independent risk factor for disease severity.It is positively correlated with IL-18 levels and eosinophil counts.Although it ex-hibits high specificity,its relatively low sensitivity suggests that it may be more suitable as an an-cillary marker for disease assessment rather than as a standalone screening tool.
