目的:比较数字化与传统稳定型咬合板治疗颞下颌关节盘不可复性前移位(anterior disc displacement without reduction,ADDWoR)的临床效果.方法:将 32 例 ADDWoR 患者随机分为传统组(n=16)与数字化组(n=16).传统组采用常规工艺制作咬合板;数字化组采用口内扫描及数字化面弓记录颌位关系与下颌运动轨迹,经计算机辅助设计与制作咬合板.评估治疗前后疼痛程度(VAS 评分),颞下颌关节功能(Fricton 指数)、口腔健康相关生存质量(OHIP-14 评分)、椅旁调改时间及患者满意度(Likert 量表)、治疗前后下颌运动范围;数字化组比较治疗前后轨迹形态.结果:治疗3 个月后,两组 Fricton 指数与 VAS 评分均较治疗前显著改善(P<0.05),但组间差异无统计学意义.数字化组椅旁调改时间显著短于传统组[(1 593.75±129.56)s vs.(2 433.75±199.56)s,P<0.001],且患者满意度评分更高(P<0.05).两组下颌运动范围较治疗前显著增大(P<0.05),数字化组运动轨迹趋于平滑对称.结论:数字化与传统稳定型咬合板在缓解 ADDWoR 核心症状上疗效相当,但数字化流程在提升诊疗效率、改善患者满意度方面更具优势.

Objective:To compare the clinical efficacy of digital versus conventional stabilization splints in the treat-ment of anterior disc displacement without reduction(ADDWoR)of the temporomandibular joint.Methods:Thirty-two ADDWoR patients were randomly allocated to a conventional group(n=16)and a digital group(n=16).The conventional group received splints made using standard laboratory procedures.The digital group underwent intraoral scanning and digital facebow recording to capture jaw relationships and mandibular movement trajectories,splints were then designed and fabrica-ted via CAD/CAM.Pain intensity(VAS score)was assessed,temporomandibular joint function(Fricton index)and oral health-related quality of life(OHIP-14 score)were assessed before and after treatment.Chairside adjustment time and patient satisfaction(Likert scale)were also recorded.The digital group was additionally evaluated for mandibular range of motion and movement trajectory morphology before and after treatment.Results:After 3 months of treatment,both groups showed signifi-cant improvement in the Fricton index and VAS scores compared to baseline(P<0.05),but no statistically significant differ-ences were found between the two groups.The chairside adjustment time in the digital group was significantly shorter than that in the conventional group[(1 593.75±129.56)s vs.(2 433.75±199.56)s,P<0.001],and patient satisfaction scores were higher in the digital group(P<0.05).Both groups demonstrated a significant increase in mandibular range of motion com-pared to baseline(P<0.05),with movement trajectories becoming smoother and more symmetrical.Conclusion:Digital and conventional stabilization splints are equally effective in alleviating the core symptoms of ADDWoR.However,the digital workflow offers advantages in improving treatment efficiency and enhancing patient satisfaction.
