目的:探究慢性牙周炎(chronic periodontitis,CP)患者血清 T 细胞免疫球蛋白黏蛋白分子 3(T cell im-munoglobulin domain and mucin domain-containing protein 3,TIM3)、Rho 相关卷曲螺旋蛋白激酶1(rho-associated coiled-coil containing protein kinase 1,ROCK1)、干扰素调节因子4(interferon regulatory factor 4,IRF4)水平与牙周临床指标的关系分析.方法:选取 2022 年 9 月~2025 年 9 月在本院口腔科收治的 216 例 CP 患者作为研究对象(CP 组)(轻度组 94 例/中重度组 122 例),另选取 216 例口腔健康成年人作为对照组.测定血清 TIM3、ROCK1、IRF4 表达水平.Pearson 相关性分析 CP 患者血清 TIM3、ROCK1、IRF4 水平与牙周临床指标、炎性因子的相关性.多因素 Logistic 回归分析 CP 患者病情严重程度的影响因素.受试者工作特征曲线(receiver operating characteristic curve,ROC)分析血清 TIM3、ROCK1、IRF4 对 CP 患者病情严重程度的评估效能.结果:CP 组血清 TIM3、ROCK1、IRF4 表达水平均显著高于对照组(P<0.05).中重度组 CP 患者牙周临床指标 PLI、SBI、PD、AL 及炎性因子 IL-6、IL-17、TNF-α 水平、TIM3、ROCK1、IRF4 表达水平均显著高于轻度组(P<0.05).血清 TIM3、ROCK1、IRF4 水平与牙周临床指标 PLI、SBI、PD、AL 及炎性因子 IL-6、IL-17、TNF-α 水平均呈正相关.血清 TIM3(OR=4.225)、ROCK1(OR=3.605)、IRF4 水平升高(OR=3.222)均是 CP 患者发生中重度病情的独立危险因素(P<0.05).血清 TIM3、ROCK1、IRF4 单独评估CP 患者病情严重程度的曲线下面积(the area under the curve,AUC)分别为 0.807、0.788 和 0.776,三者联合评估AUC 为 0.927,灵敏度为 86.07%,特异度为 91.49%,优于单独评估(P<0.05).结论:CP 患者血清 TIM3、ROCK1、IRF4 水平升高,且与 PD、AL、GBI 等牙周临床指标及炎性因子密切相关,三者联合检测对评估病情具有较高效能.

Objective:To explore the relationship between serum T-cell immunoglobulin and mucin domain-contai-ning molecule 3(TIM3),rho-associated coiled-coil containing protein kinase 1(ROCK1),interferon regulatory factor 4(IRF4)levels and periodontal clinical indicators in patients with chronic periodontitis(CP).Methods:Totally 216 CP pa-tients were selected as the research subjects(CP group)(mild group 94 cases,moderate-severe group 122 cases)based on periodontal clinical indicators.Additionally,216 healthy adults with oral health were selected as the control group.The ex-pression levels of TIM3,ROCK1 and IRF4 in serum were determined.Pearson correlation was used to analyze the correlations between serum TIM3,ROCK1 and IRF4 levels and periodontal clinical indicators,inflammatory factors in CP patients.Multi-variate Logistic regression analysis was conducted to identify the influencing factors of the severity of the condition in CP pa-tients.Moreover,receiver operating characteristic curve(ROC)was used to analyze the efficiency of serum TIM3,ROCK1,and IRF4 in assessing the severity of the condition in CP patients.Results:The serum TIM3,ROCK1 and IRF4 in the CP group were obviously higher than those in the control group(P<0.05).The periodontal clinical indicators(PLI,SBI,PD,AL)and inflammatory factors(IL-6,IL-17,TNF-α),TIM3,ROCK1,and IRF4 of CP patients in the moderate-severe group were obviously higher than those in the mild group(P<0.05).Serum TIM3,ROCK1 and IRF4 were positively correlated with the clinical indicators of periodontal disease(PLI,SBI,PD,AL)and inflammatory factors(IL-6,IL-17,TNF-α).The elevated ser-um TIM3(OR=4.225),elevated ROCK1(OR=3.605),and elevated IRF4(OR=3.222)were independent risk factors for the development of moderate to severe disease in CP patients(P<0.05).The area under the curve(AUC)for the individual assessment of the severity of CP patients by serum TIM3,ROCK1,and IRF4 were 0.807,0.788,and 0.776 respectively.The AUC for the combined assessment of the three was 0.927,with a sensitivity of 86.07%and a specificity of 91.49%,com-bined assessment was superior to individual assessments(P<0.05).Conclusion:Serum TIM3,ROCK1 and IRF4 in CP pa-tients are elevated,and they are closely related to dental periodontal clinical indicators such as PD,AL and GBI,as well as inflammatory factors.The combined detection of these three indicators has a relatively high efficiency in assessing the condi-tion.
