目的:探讨术前锥形束 CT(cone beam computer tomography,CBCT)测量的骨密度 Hounsfield Unit(HU)值与后牙区常规种植体初期稳定性、二期手术需求的相关性,明确二期手术需求的 HU 值量化预警区间.方法:回顾性分析2025 年3 月~9 月我院62 例后牙区单颗常规种植患者的临床资料,按术后是否需要二期手术分组,分析各区段 HU 值与二期手术需求的关联,受试者工作特征曲线(receiver operating characteristic curve,ROC)确定 HU 值临界值及预警区间.结果:需要二期手术组各区段 HU 值均显著低于无需二期手术组(P<0.001),根部 HU 值相关性最强(r=-0.712);平均 HU 值、上颌后牙区是二期手术需求的独立预测因子(P<0.05).平均 HU 值预测二期手术需求的曲线下面积(the area under the curve,AUC)为 0.897,最佳临界值 1250 HU;种植体品牌、规格对其无显著影响(P>0.05).结论:术前 CBCT 测量的 HU 值可判断后牙区种植体初期稳定性及二期手术需求(根部 HU 值预测最佳),其分级预警区间结合上颌后牙区高风险特征可指导干预治疗方案,低HU 值下调整种植体品牌、规格无明显作用.

Objective:To investigate the correlation between bone mineral density Hounsfield Unit(HU)values measured by preoperative cone beam computer tomography(CBCT)and primary stability as well as the need for secondary surgery of conventional implants in the posterior dental region,define the quantitative early warning interval of HU values for the need for secondary surgery.Methods:The clinical data of 62 patients who received single conventional implant placement in the posterior dental region in our hospital from March to September 2025 were retrospectively analyzed.The patients were divided into groups according to whether secondary surgery was required after implantation.Multiple statistical methods were used to analyze the correlation between HU values in different segments and the need for secondary surgery,and the receiver operating characteristic(ROC)curve was adopted to determine the critical value and early warning interval of HU values.Re-sults:HU values in all segments of the group requiring secondary surgery were significantly lower than those of the group without secondary surgery(P<0.001),and the HU values at the implant root showed the strongest correlation(r=-0.712).Mean HU values and the maxillary posterior dental region were independent predictors for the need for secondary surgery(P<0.05).The area under the curve(AUC)of mean HU values for predicting the need for secondary surgery was 0.897,with the optimal critical value of 1250 HU.Implant brand and specification had no significant effect on the need for secondary surgery(P>0.05).Conclusion:HU values measured on preoperative CBCT can predict the primary stability of dental implants and the need for secondary surgery in the posterior dentition,with apical HU values showing the best predictive performance.The established grading warning threshold for HU values,combined with the high-risk characteristics of the posterior maxilla,can guide intervention strategies.Adjusting implant brand or dimensions shows no significant benefit under low HU conditions.
