CGF与Bio-Oss不同比例复合物引导颌骨再生的临床效果评价OA
Clinical evaluation of different proportions of CGF and Bio-Oss composites in guiding jawbone regeneration
目的:评估浓缩生长因子(concentrated growth factors,CGF)与Bio-Oss 骨粉不同比例复合物在引导颌骨再生中的临床效果,并探讨其最优复合比例.方法:选取 40 例颌骨囊肿患者按随机数字表法分为 4 组,分别使用CGF 与 Bio-Oss 按质量比 1∶1、2∶1、4∶1 以及全 CGF 的比例制备复合材料植入骨腔内进行颌骨再生修复,分别对应A、B、C 3 组实验组和 D 对照组.术后 3 个月、6 个月评估骨再生情况,比较不同复合比例组临床效果.结果:术后遗留骨腔的平均灰度值在所有观察时间点,A、B、C 3 组骨再生效果均显著优于 D 组(P<0.05),术后 6 个月时 B 组骨缺损区灰度值显著高于 A、C 两组(P<0.05);术后炎症程度及肿胀程度方面实验组 B、C 组指标数值均显著低于 A组(P<0.05),同时C 组低于B 组,但两组间无显著差异(P>0.05).术后6 个月组织学图像显示Bio-Oss 骨粉颗粒基本消失及大量新生骨和骨细胞形成,骨小梁排列紧密,骨成熟度高.结论:术后 6 个月时 CGF 与 Bio-Oss 质量比 2∶1在促进颌骨再生方面具有最佳骨再生效果,并加快患者术后恢复速度.
Objective:To assess the clinical efficacy of different CGF(concentrated growth factor)and Bio-Oss bone meal composite ratios in guiding jawbone regeneration and determine the optimal ratio.Methods:Forty patients with jawbone cysts were divided into 4 groups.Composite materials were prepared with CGF and Bio-Oss at mass ratios of 1∶1(group A),2∶1(group B),4∶1(group C),and pure CGF(control group D),then implanted into bone cavities for regeneration.Follow-ups included imaging examinations and assessment of postoperative inflammation/swelling.Bone regeneration was evaluated at 3 and 6 months postoperatively.Results:At all observation time points,the average gray value of the residual bone cavity after jaw cyst surgery,bone regeneration in groups A,B,and C was significantly better than in group D(P<0.05).At 6 months,group B had significantly higher values than A,C(P<0.05).For inflammation/swelling,group B and C had lower indices than group A(P<0.05),group C lower than group B(P>0.05).Histological evaluations at 6 months postoperatively demon-strated nearly complete resorption of Bio-Oss bone graft particles,robust neo-osteogenesis and osteocyte formation,as well as dense,well-organized bone trabeculae with high osteogenic maturity.Conclusion:A CGF-to-Bio-Oss mass ratio of 2∶1 opti-mizes jawbone regeneration at 6 months,enhancing bone repair and accelerating patient recovery.
