目的 探讨骨质疏松症(OP)患者血清小窝蛋白1(CAV1)、富含半胱氨酸的血管生成诱导因子61(CYR61)、高迁移率族蛋白 A2(HMGA2)水平及对其发生骨折的预测价值.方法 选取2023年1月至2024年5月该院收治的143例 OP患者作为研究组,根据1年内是否发生骨折分为未骨折组和骨折组;另选取同期在该院体检的143例健康体检者作为对照组.收集所有研究对象基线资料.检测所有研究对象血清Ⅰ型前胶原氨基末端肽(PⅠNP)、骨钙素(BGP)、Ⅰ型胶原C末端肽(CTX)、CAV1、CYR61、HMGA2水平.检测 OP患者骨密度,采用T值表示.采用相对危险度(RR)评估不同CAV1、CYR61、HMGA2水平的 OP患者与骨折风险的关系;绘制受试者工作特征(ROC)曲线分析血清CAV1、CYR61、HMGA2单独及三者联合检测对 OP患者发生骨折的预测价值;采用Pearson相关分析 OP患者血清CAV1、CYR61、HMGA2水平与骨密度的相关性.结果 研究组血清CAV1、CYR61、HMGA2水平均高于对照组,差异均有统计学意义(P<0.05).骨折组72例,未骨折组71例,骨折组T值及血清PINP、BGP、CAV1、CYR61、HMGA2水平均低于未骨折组,CTX水平高于未骨折组,差异均有统计学意义(P<0.05).相对危险度分析结果显示,低水平CAV1、CYR61、HM-GA2的 OP患者发生骨折的风险高于高水平CAV1、CYR61、HMGA2的 OP患者.ROC曲线分析结果显示,CAV1、CYR61、HMGA2单独预测 OP患者发生骨折的曲线下面积(AUC)分别为0.872、0.796、0.862,三者联合预测 OP患者发生骨折的 AUC(0.966)大于CAV1、CYR61、HMGA2单独预测的 AUC(Z=3.616、4.734、3.560,P<0.001).Pearson相关分析结果显示,OP患者血清 CAV1、CYR61、HMGA2水平与骨密度及PINP、BGP水平均呈正相关(P<0.05),与CTX水平均呈负相关(P<0.05).结论 OP患者血清 CAV1、CYR61、HMGA2水平均降低,对骨折发生具有一定预测价值,且三者联合预测价值更高.

Objective To investigate the serum levels of caveolin-1(CAV1),cysteine-rich angiogenesis in-ducing factor 61(CYR61)and high mobility group A2(HMGA2)and their predictive value for fracture in pa-tients with osteoporosis(OP).Methods A total of 143 OP patients admitted to the hospital from January 2023 to May 2024 were selected as the study group,and were divided into non-fracture group and fracture group according to whether fracture occurred within 1 year.In addition,143 healthy people who underwent physical examination in the same hospital during the same period were selected as the control group.Baseline data of all subjects were collected.The levels of serum N-terminal procollagen type Ⅰ(PⅠNP),osteocalcin(BGP),C-terminal telopeptide of type I collagen(CTX),CAV1,CYR61 and HMGA2 were detected in all sub-jects.Bone mineral density(BMD)was measured and expressed as T value.Relative risk(RR)was used to e-valuate the relationship between OP patients with different levels of CAV1,CYR61,HMGA2 levels and frac-ture risk.The receiver operating characteristic(ROC)curve was drawn to analyze the predictive value of ser-um CAV1,CYR61 and HMGA2 alone and in combination for fracture in OP patients.Pearson correlation was used to analyze the correlation between serum CAV1,CYR61,HMGA2 levels and bone mineral density in OP patients.Results The levels of CAV1,CYR61 and HMGA2 in the study group were higher than those in the control group,and the differences were statistically significant(P<0.05).There were 72 cases in the fracture group and 71 cases in the non-fracture group.The T value and serum PINP,BGP,CAV1,CYR61 and HMGA2 levels in the fracture group were lower than those in the non-fracture group,and the CTX level was higher than that in the non-fracture group,and the differences were statistically significant(P<0.05).The relative risk analysis showed that the risk of fracture in OP patients with low levels of CAV1,CYR61 and HMGA2 was higher than that in OP patients with high levels of CAV1,CYR61 and HMGA2.ROC curve analysis showed that the area under the curve(AUC)of CAV1,CYR61 and HMGA2 alone in predicting fracture in OP patients was 0.872,0.796 and 0.862 respectively.The AUC of the combination of CAV1,CYR61 and HM-GA2 in predicting fracture in OP patients(0.966)was greater than that of CAV1,Cyr61 and HMGA2 alone(Z=3.616,4.734,3.560,P<0.001).Pearson correlation analysis showed that the serum levels of CAV1,CYR61 and HMGA2 in OP patients were positively correlated with bone mineral density,PINP and BGP lev-els(P<0.05),and negatively correlated with CTX levels(P<0.05).Conclusion The serum levels of CAV1,CYR61 and HMGA2 in OP patients are decreased,which have certain predictive value for fracture oc-currence,and the combined predictive value of the three is higher.
