Diagnostic value of serum Tim-1 and CCL19 in allergic rhinitis

目的 探讨血清 T 淋巴细胞免疫球蛋白域黏蛋白域蛋白-1(Tim-1)与 CC趋化因子配体19(CCL19)对变应性鼻炎(AR)的诊断价值.方法 选取2020年5月至2023年7月在该院接受诊治的126例AR患者作为AR组,根据病情严重程度将 AR组分为轻度组(41例)和中重度组(85例).另选取同期在该院进行体检的126例健康体检者作为对照组.检测所有研究对象血清Tim-1、CCL19水平;采用Pearson相关分析AR患者血清Tim-1、CCL19水平与免疫球蛋白(Ig)E水平及过敏性鼻炎评分量表(SFAR)评分的相关性,以及血清Tim-1水平与CCL19水平的相关性.采用多因素Logistic回归分析发生AR的影响因素.绘制受试者工作特征(ROC)曲线分析血清Tim-1、CCL19单独及二者联合检测对 AR的诊断价值.结果 AR组血清Tim-1、CCL19水平均高于对照组,差异均有统计学意义(P<0.05).中重度组血清Tim-1、CCL19水平均高于轻度组,差异均有统计学意义(P<0.05).IgE阳性、SFAR评分≥7分的 AR患者血清 Tim-1、CCL19水平均高于IgE阴性、SFAR评分<7分的 AR患者,差异均有统计学意义(P<0.05).Pearson相关分析结果显示,血清Tim-1、CCL19水平与IgE水平、SFAR评分均呈正相关(P<0.05),且血清 Tim-1水平与CCL19水平呈正相关(P<0.05).多因素Logistic回归分析结果显示,Tim-1、CCL19水平升高均为发生 AR的独立危险因素(P<0.05).ROC曲线分析结果显示,Tim-1、CCL19单独及二者联合诊断AR的曲线下面积(AUC)分别为0.749、0.752、0.847,二者联合诊断的 AUC大于 Tim-1、CCL19单独诊断的 AUC(Z=3.782、3.886,P<0.05).结论 AR患者血清Tim-1、CCL19水平均升高,且与病情严重程度、IgE水平及SFAR评分密切相关,提示二者可能作为辅助诊断AR的潜在血清学标志物.

Objective To investigate the diagnostic value of serum T lymphocyte immunoglobulin domain mucin domain-1(Tim-1)and CC chemokine ligand 19(CCL19)in allergic rhinitis(AR).Methods A total of 126 patients with AR who were diagnosed and treated in this hospital from May 2020 to July 2023 were select-ed as the AR group.According to the severity of the disease,the AR group was divided into mild group(41 ca-ses)and moderate to severe group(85 cases).Another 126 healthy people who underwent physical examina-tion in the hospital during the same period were selected as the control group.The serum levels of Tim-1 and CCL19 were detected in all subjects.Pearson correlation was used to analyze the correlation of serum Tim-1 and CCL19 levels with immune globulin(Ig)E level and score for allergic rhinits(SFAR)score,and the corre-lation between serum Tim-1 level and CCL19 level in AR patients.Multivariate Logistic regression was used to analyze the influencing factors of AR.The receiver operating characteristic(ROC)curve was drawn to ana-lyze the diagnostic value of serum Tim-1,CCL19 alone and combined detection for AR.Results The serum levels of Tim-1 and CCL19 in AR group were higher than those in control group,and the differences were sta-tistically significant(P<0.05).The serum levels of Tim-1 and CCL19 in the moderate to severe group were higher than those in the mild group,and the differences were statistically significant(P<0.05).The serum levels of Tim-1 and CCL19 in IgE positive AR patients with SFAR score≥7 were higher than those in IgE negative AR patients with SFAR score<7,and the differences were statistically significant(P<0.05).Pear-son correlation analysis showed that serum Tim-1 and CCL19 levels were positively correlated with IgE level and SFAR score(P<0.05),and serum Tim-1 level was positively correlated with CCL19 level(P<0.05).Multivariate Logistic regression analysis showed that increased levels of Tim-1 and CCL19 were independent risk factors for AR(P<0.05).The results of ROC curve analysis showed that the area under the curve(AUC)of Tim-1,CCL19 alone and in combination in the diagnosis of AR were 0.749,0.752 and 0.847 re-spectively.The AUC of combined diagnosis was greater than that of Tim-1 and CCL19 alone(Z=3.782,3.886,P<0.05).Conclusion The serum levels of Tim-1 and CCL19 in AR patients are increased,and are closely related to the severity of the disease,IgE level and SFAR score,suggesting that TIM-1 and CCL19 may be used as potential serological markers for auxiliary diagnosis of AR.
