等离子射频消融术联合口服盐杜仲颗粒治疗腰椎间盘突出症的临床疗效OA
Clinical efficacy of plasma radiofrequency ablation combined with oral salt Eucommia granules in the treatment of lumbar disc herniation
目的 探讨等离子射频消融术(RFA)联合口服盐杜仲颗粒治疗腰椎间盘突出症(LDH)的临床疗效.方法 选取2022年1月至2024年12月该院收治的208例LDH 患者作为研究对象,采用随机数字表法分为射频组(103例)与联合颗粒组(105例).射频组采用低温 RFA进行治疗,联合颗粒组在射频组治疗基础上联合盐杜仲颗粒进行治疗.比较2组临床疗效、疼痛程度、腰椎功能及血清学指标[β-内啡肽(β-EP)、基质金属蛋白酶-3(MMP-3)、前列腺素E2(PGE2)]水平.统计2组治疗期间出现的不良反应.结果 联合颗粒组总有效率为98.10%,大于射频组的86.41%,差异有统计学意义(P<0.05).治疗后,2组视觉模拟评分法(VAS)评分及血清 MMP-3、PGE2 水平均低于治疗前,血清β-EP水平高于治疗前,且联合颗粒组 VAS评分及血清 MMP-3、PGE2 水平均低于射频组,血清β-EP水平高于射频组,联合颗粒组 VAS评分及血清 MMP-3、PGE2、β-EP水平差值的绝对值均大于射频组,差异均有统计学意义(P<0.05).治疗后,2组血清β-EP水平、主观症状、临床体征、活动受限度JOA评分及总分均高于治疗前,且联合颗粒组均高于射频组,联合颗粒组血清β-EP水平、主观症状、临床体征、活动受限度JOA评分及总分差值的绝对值均大于射频组,差异均有统计学意义(P<0.05).治疗期间,2组均未发生皮疹、瘙痒等不良反应.结论 RFA联合盐杜仲颗粒能够有效减轻患者的腰腿疼痛,提高患者腰椎功能,优化 MMP-3、PGE2、β-EP指标水平,提高临床疗效.
Objective To investigate the clinical efficacy of plasma radiofrequency ablation(RFA)com-bined with oral salt eucommia granules in the treatment of lumbar disc herniation(LDH).Methods A total of 208 patients with LDH admitted to this hospital from January 2022 to December 2024 were selected as the research objects,and they were divided into radiofrequency group(103 cases)and combined particle group(105 cases)by random number table method.The radiofrequency group was treated with low temperature RFA,and the combined granule group was treated with salt Eucommia granule on the basis of the radiofre-quency group.The clinical efficacy,pain degree,lumbar function and serological indexes[β-endorphin(β-EP),matrix metalloproteinase-3(MMP-3),prostaglandin E2(PGE2)]levels were compared between the two groups.The adverse reactions of the two groups during treatment were recorded.Results The total effective rate of the combined particle group was 98.10%,which was higher than 86.41%of the radiofrequency group,and the difference was statistically significant(P<0.05).After treatment,the visual analogue scale(VAS)score,serum MMP-3 and PGE2 levels of the two groups were lower than those before treatment,and the ser-um β-EP level was higher than that before treatment.The VAS score,serum MMP-3 and PGE2 levels of the combined granule group were lower than those of the radiofrequency group,and the serum β-EP level was higher than that of the radiofrequency group,the absolute value of VAS score and the difference of serum MMP-3,PGE2 and β-EP levels in the combined granule group were greater than those in the radiofrequency group,and the differences were statistically significant(P<0.05).After treatment,the serum levels of β-EP,subjective symptoms,clinical signs,JOA score and total score of activity limitation in both groups were higher than those before treatment,and those in the combined granule group were higher than those in the radiofre-quency group.The absolute values of the differences in the levels of β-EP,subjective symptoms,clinical signs,JOA score and total score of activity limitation in the combined granule group were greater than those in the radiofrequency group,the differences were statistically significant(P<0.05).During the treatment,no ad-verse reactions such as rashes or itching occurred in the two groups.Conclusion RFA combined with salt eu-commia granules can effectively reduce the patients'low back pain and leg pain,improve the patients'lumbar function,optimize MMP-3,PGE2,β-EP index levels,and improve the clinical efficacy.
