IBI、SAA/hs-CRP比值对重症腺病毒肺炎患儿预后的影响OA
Effects of IBI and SAA/hs-CRP ratio on the prognosis of children with severe adenovirus pneumonia
目的 探讨炎症负担指数(IBI)、血清淀粉样蛋白 A/超敏C反应蛋白(SAA/hs-CRP)比值对重症腺病毒肺炎(SAdVP)患儿预后的影响.方法 选取2023年5月至2025年5月西安工会医院收治的175例SAdVP患儿作为SAdVP组,另选取同期西安工会医院收治的175例轻症腺病毒肺炎(MAdVP)患儿作为MAdVP组和同期在西安工会医院体检的175例健康儿童作为对照组.计算所有研究对象IBI、SAA/hs-CRP比值.根据SAdVP患儿预后将其分为不良组和良好组.采用多因素Logistic回归分析SAdVP患儿预后不良的影响因素;绘制受试者工作特征(ROC)曲线分析IBI、SAA/hs-CRP比值单独及二者联合对SAdVP患儿预后不良的预测效能.结果 SAdVP组IBI、SAA/hs-CRP比值均高于 MAdVP组、对照组,且 MAdVP组IBI、SAA/hs-CRP比值均高于对照组,差异均有统计学意义(P<0.05).良好组123例,不良组52例.不良组发生ARDS患者比例、发生循环系统并发症患者比例、IBI、SAA/hs-CRP比值及降钙素原水平均高于良好组,差异均有统计学意义(P<0.05).多因素Logistic回归分析结果显示,发生ARDS、发生循环系统并发症、IBI升高、SAA/hs-CRP比值升高均为SAdVP患儿预后不良的独立危险因素(P<0.05).ROC曲线分析结果显示,IBI、SAA/hs-CRP比值单独及二者联合预测SAdVP患儿预后不良的曲线下面积(AUC)分别为0.812、0.801、0.893,二者联合预测SAdVP患儿预后不良的AUC大于IBI、SAA/hs-CRP比值单独预测的AUC(Z=3.277、3.196,P<0.05).结论 SAdVP患儿IBI、SAA/hs-CRP比值均升高,且均为SAdVP患儿预后不良的独立危险因素,IBI、SAA/hs-CRP比值联合对SAdVP患儿预后不良的预测效能较高.
Objective To investigate the effects of inflammatory burden index(IBI)and serum amyloid A/high-sensitivity C reactive protein(SAA/hs-CRP)ratio on the prognosis of children with severe adenovirus pneumonia(SAdVP).Methods A total of 175 children with SAdVP admitted to Xi'an Union Hospital from May 2023 to May 2025 were selected as the SAdVP group,175 children with mild adenovirus pneumonia(MAdVP)admitted during the same period in Xi'an Union Hospital were selected as the MAdVP group,and 175 healthy children who underwent physical examinations at Xi'an Union Hospital were selected as the con-trol group.IBI and SAA/hs-CRP ratio were calculated for all subjects.According to the prognosis of children with SAdVP,they were divided into poor group and good group.Multivariate Logistic regression was used to analyze the influencing factors of poor prognosis in children with SAdVP.The receiver operating characteristic(ROC)curve was drawn to analyze the predictive efficacy of IBI and SAA/hs-CRP ratio alone and in combina-tion for the poor prognosis of children with SAdVP.Results The IBI and SAA/hs-CRP ratio of the SAdVP group were higher than those of the MAdVP group and the control group,and the IBI and SAA/hs-CRP ratio of the MAdVP group were higher than those of the control group,and the differences were statistically signifi-cant(P<0.05).There were 123 cases in the good group and 52 cases in the poor group.The proportion of ARDS patients,the proportion of patients with circulatory system complications,IBI,SAA/hs-CRP ratio,pro-calcitonin levels in the poor group were higher than those in the good group,and the differences were statisti-cally significant(P<0.05).The results of multivariate Logistic regression analysis showed that ARDS,circu-latory system complications,increased IBI,and increased SAA/hs-CRP ratio were independent risk factors for poor prognosis in children with SAdVP(P<0.05).ROC curve analysis showed that the area under the curve(AUC)of IBI,SAA/hs-CRP ratio alone and their combination for predicting the poor prognosis of children with SAdVP was 0.812,0.801 and 0.893 respectively.The AUC of the combination of the two to predict the poor prognosis of children with SAdVP was greater than that of IBI or SAA/hs-CRP ratio alone(Z=3.277,3.196,P<0.05).Conclusion IBI and SAA/hs-CRP ratio are increased in children with SAdVP,and they are both independent risk factors for poor prognosis of children with SAdVP.The combination of IBI and SAA/hs-CRP ratio has a high predictive efficiency for the poor prognosis of children with SAdVP.
