目的 评估恶性肿瘤患儿的营养不良风险,并分析其相关影响因素.方法 选取 2022 年 11 月—2023 年 3 月上海交通大学医学院附属新华医院儿童血液肿瘤科收治的 177 例年龄 2 个月~17 岁的恶性肿瘤患儿.采用儿科营养不良评估筛查工具(screening tool for assessment of malnutrition in pediatrics,STAMP),从疾病风险、膳食摄入及生长发育 3 个维度对患儿进行评估,依据 STAMP 总分判断患儿是否存在营养不良风险,并采用多因素二元Logistic 回归模型分析其影响因素.结果 177 例恶性肿瘤患儿中,9 例(5.08%)存在高度营养不良风险,85 例(48.02%)存在中度营养不良风险,83 例(46.89%)存在低度或无营养不良风险.不同年龄、肿瘤分期、化疗次数患儿的 STAMP 评分差异有统计学意义(均 P<0.05).Logistic 回归分析显示,以 STAMP 评分≥2 分为营养不良风险界限,患儿年龄[OR(95%CI):0.14(0.02~0.96)]、化疗次数[OR(95%CI):1.04(1.00~1.08)]及入院时血清白蛋白水平[OR(95%CI):0.29(0.11~0.77)]与营养不良风险显著相关(均P<0.05).结论 恶性肿瘤患儿营养不良风险较高,其中年龄偏小、化疗次数较多的患儿尤为突出.联合营养不良风险筛查与血清白蛋白水平检测,可尽早识别营养不良风险,进而采取合理的营养支持措施,改善患儿生活质量,提升疾病治疗效果.

Objective To assess the malnutrition risk in children with malignant tumors,and analyze its related influencing factors.Methods A total of 177 children aged 2 months to 17 years with malignant tumors admitted to the Department of Pediatric Hematology and Oncology,Xinhua Hospital,Shanghai Jiao Tong University School of Medicine from November 2022 to March 2023 were enrolled.The screening tool for assessment of malnutrition in pediatrics(STAMP)was used to evaluate the children from three dimensions:disease risk,dietary intake,and growth and development.Malnutrition risk was determined according to the total STAMP score,and multivariate binary Logistic regression was used to analyze the influencing factors.Results Among the 177 children,9(5.08%)had high malnutrition risk,85(48.02%)had moderate risk,and 83(46.89%)had low or no risk.STAMP scores differed significantly by age,tumor stage,and number of chemotherapy cycles(all P<0.05).Logistic regression analysis,using a STAMP score≥2 as the cutoff for malnutrition risk,showed that age[OR(95%CI):0.14(0.02 to 0.96)],number of chemotherapy cycles[OR(95%CI):1.04(1.00 to 1.08)],and serum albumin level at admission[OR(95%CI):0.29(0.11 to 0.77)]were significantly associated with malnutrition risk(all P<0.05).Conclusion Children with malignant tumors are at high malnutrition risk,especially younger children and those with more chemotherapy cycles.Combining malnutrition risk screening with serum albumin measurement allows for early identification of malnutrition risk and timely implementation of nutritional support,thereby improving quality of life and treatment outcomes.
