目的 分析 2~6 岁室间隔缺损(ventricular septal defect,VSD)患儿的运动发育特征,为制定个体化干预方案提供理论依据.方法 采用回顾性研究,纳入 123 例 2~6 岁 VSD 患儿(VSD 组),按病例与对照人数2 1 的比例,匹配性别、年龄相符的 62例健康儿童作为对照组.采用Peabody 运动发育量表(第 2 版)(Peabody developmental motor scale-second edition,PDMS-2)对两组儿童进行测评,记录原始分并计算总运动发育商(total motor quotient,TMQ)、粗大运动发育商(gross motor quotient,GMQ)、精细运动发育商(fine motor quotient,FMQ),以及粗大运动中姿势、移动、实物操作能区标准分与精细运动中抓握、视觉运动整合能区标准分.收集数据后进行统计学分析.结果 VSD 组儿童 TMQ、GMQ 与 FMQ 均显著低于对照组儿童(P<0.05);对两组儿童粗大运动和精细运动各能区标准分的分析发现,VSD 组儿童姿势、移动、实物操作及视觉运动整合标准分均显著低于对照组(P<0.05),两组间抓握能区标准分的差异无统计学意义(P>0.05).结论 2~6 岁 VSD 患儿的运动发育总体水平显著低于同龄健康儿童.早期干预可通过姿势控制、实物操作能力训练促进其粗大运动发育,通过视觉运动整合能力训练促进其精细运动发育.

Objective To analyze the motor development characteristics of children aged 2 to 6 years with ventricular septal defect(VSD),and provide a theoretical basis for formulating individualized intervention programs.Methods A retrospective study was conducted,enrolling 123 children aged 2 to 6 years with VSD(VSD group).According to a case-to-control ratio of 2∶1,62 healthy children matched for gender and age were included as the control group.All children of both groups were evaluated using the Peabody developmental motor scale-second edition(PDMS-2).Raw scores were recorded,and the total motor quotient(TMQ),gross motor quotient(GMQ),and fine motor quotient(FMQ)were calculated.Standard scores for the gross motor subdomains(posture,mobility,and object manipulation)and fine motor subdomains(grasping and visual-motor integration)were also calculated.The collected data were statistically analyzed.Results The TMQ,GMQ,and FMQ were significantly lower in the VSD group than in the control group(P<0.05).Analysis of the standard scores for each gross and fine motor subdomain showed that posture,mobility,object manipulation,and visual-motor integration scores were significantly lower in the VSD group than in the control group(P<0.05),whereas no significant difference was found in grasping scores between the two groups(P>0.05).Conclusion The overall motor development of children aged 2 to 6 years with VSD is significantly poorer than that of age-matched healthy children.Early intervention can promote gross motor development through training in postural control and object manipulation,and enhance fine motor development through visual-motor integration training.
