This article summarizes Professor Zhou Xiaoling's clinical experience in the syndrome differentiation and treatment of liver cirrhosis based on its staged pathogenesis of"co-existence of qi deficiency and blood stasis".In accordance with the clinical characteristics of liver cirrhosis,Zhou Xiaoling outlines its core pathogenesis as"co-existence of qi deficiency and blood stasis",and proposes a holistic TCM intervention framework of"whole-process staged syndrome differentiation and treatment".The approach adheres to the principle of"tonifying qi deficiency and removing blood stasis",tailored to each phase of the disease progression.In the early stage,liver cirrhosis is commonly characterized by liver-spleen disharmony and spleen yang deficiency,and she treats it with modified Lizhong Decoction to soothe the liver,strengthen the spleen,and resolve dampness.In the middle stage,the disease is marked by liver-kidney deficiency and stasis obstructing collaterals,and she treats it with modified Jisheng Shenqi Decoction to tonify the kidney,regenerate the liver,and resolve stasis.While in the advanced stage,the disease features liver-spleen-kidney deficiency,intermingled cold and heat,and co-existence of qi deficiency and blood stasis,and she treats it with modified Wenjing Decoction,focusing on harmonizing yin and yang by adopting both warming and clearing methods to nourish blood while resolving stasis.She advocates applying classical formular flexibly in clinical practice,underlining harmonizing the liver,spleen and kidney,balancing qi,blood,yin and yang.Professor Zhou Xiaoling follows the principle of"resolving stasis without impairing healthy qi,tonifying deficiency without retaining stasis"throughout the different stages,developing a stepwise diagnostic and therapeutic paradigm that integrates the disease,its syndromes and symptoms.This approach has demonstrated significant clinical efficacy.
