目的:旨在探讨真武汤对糖尿病肾病(DN)大鼠肾脏损伤的改善作用及机制.方法:通过腹腔注射链脲佐菌素(STZ)构建 DN 大鼠模型.将造模成功的大鼠随机分为模型组、恩格列净组(灌胃10mg/kg 恩格列净混悬液)、真武汤组(灌胃 25.20g/kg 的真武汤)和真武汤+通路激活剂组(灌胃 25.20g/kg 的真武汤+10mg/kg 的 SKL2001 溶液),15 只正常大鼠作为对照组.使用血糖仪检测大鼠血糖水平;全自动生化分析仪检测大鼠 24h 尿蛋白、血肌酐(SCR)、尿素氮(BUN)水平;苏木精-伊红(HE)染色、过碘酸希夫(PAS)染色和马松三色(Masson)染色分析大鼠肾组织病理学变化;实时荧光定量聚合酶链反应(RT-qPCR)检测肾组织中单核细胞趋化蛋白 1(MCP-1)、肿瘤坏死因子 α(TNF-α)、白细胞介素1β(IL-1β)、核因子 Iκ 抑制蛋白 α(IκBα)mRNA 水平;Western blotting 检测 Wnt/β-catenin 信号通路相关蛋白表达水平.结果:模型组大鼠肾组织病理损伤明显,糖原沉积增加,血糖、24h 尿蛋白、SCR、BUN、MCP-1、TNF-α、IL-1β mRNA、胶原蛋白沉积相对面积、FN、Col I、Wnt4、p-GSK3β、β-cate-nin、α-SMA 水平较对照组大幅度上调(P<0.05),IκBα mRNA 水平较对照组大幅度下调(P<0.05);恩格列净组、真武汤组和真武汤+通路激活剂组大鼠肾组织病理损伤有所缓解,血糖、24h 尿蛋白、SCR、BUN、MCP-1、TNF-α、IL-1β mRNA、胶原蛋白沉积相对面积、FN、Col I、Wnt4、p-GSK3β、β-catenin、α-SMA 水平较模型组大幅度下调(P<0.05),IκBα mRNA 水平较模型组大幅度上调(P<0.05);真武汤组和真武汤+通路激活剂组血糖、24h 尿蛋白、SCR、BUN、MCP-1、TNF-α、IL-1β mRNA、胶原蛋白沉积相对面积、FN、Col I、Wnt4、p-GSK3β、β-catenin、α-SMA 水平较恩格列净组大幅度下调(P<0.05),IκBα mRNA 水平较恩格列净组大幅度上调(P<0.05);真武汤+通路激活剂组血糖、24h 尿蛋白、SCR、BUN、MCP-1、TNF-α、IL-1β mRNA、蓝色胶原蛋白沉积、FN、Col I、Wnt4、p-GSK3β、β-catenin、α-SMA 水平较真武汤组大幅度上调(P<0.05),IκBα mRNA 水平较真武汤组大幅度下调(P<0.05).结论:真武汤能够缓解 DN 大鼠肾损伤,抑制炎症反应,改善肾功能和肾脏纤维化,其机制可能与抑制 Wnt/β-catenin信号通路有关.

Objective:To investigate the protective effect of Zhenwu Decoction on renal injury in rats with diabetic nephropathy(DN)and its underlying mechanism.Methods:A DN rat model was established by intraperitoneal injection of streptozotocin(STZ).The successfully modeled rats were randomly divided into a model group,an empagliflozin group(10mg/kg empagliflozin suspension by gavage),a Zhenwu decoction group(25.20g/kg Zhenwu decoction by gavage)and a Zhenwu decoction+pathway activator group(25.20g/kg Zhenwu decoction+10mg/kg SKL2001 solution by gavage),and another 15 normal rats served as the con-trol group.Blood glucose levels were measured with a glucometer.Urinary protein over 24 hours,serum creat-inine(SCR),and blood urea nitrogen(BUN)were detected using an automatic biochemical analyzer.Renal histopathological changes were assessed by hematoxylin-eosin(HE)staining,periodic acid-Schiff(PAS)staining,and Masson's trichrome staining.mRNA levels of monocyte chemoattractant protein-1(MCP-1),tumor necrosis factor-α(TNF-α),interleukin-1β(IL-1β),and nuclear factor-κB inhibitor α(IκBα)in renal tissue were measured by RT-qPCR.Protein expression levels of components of the Wnt/β-catenin sig-naling pathway were detected by Western blotting.Results:The kidney tissue of the model group rats showed significant pathological injury and increased glycogen deposition.The levels of blood glucose,24-hour urinary protein,SCR(serum creatinine),BUN(blood urea nitrogen),MCP-1(monocyte chemoattractant protein-1),TNF-α(tumor necrosis factor-alpha),IL-1β mRNA,blue collagen deposition,FN(fibronectin),Col I(collagen type I),Wnt4,p-GSK3β(phosphorylated glycogen synthase kinase-3 beta),β-catenin,and α-SMA(alpha-smooth muscle actin)were significantly upregulated compared to the control group(P<0.05),while the level of IκBα mRNA was significantly lower(P<0.05).The empagliflozin,Zhenwu Decoction,and Zhenwu Decoction+pathway activator groups all exhibited alleviated renal pathological injury,with signifi-cantly lower levels of the above parameters and higher IκBα mRNA levels compared with the model group(P<0.05).Compared with the empagliflozin group,the Zhenwu Decoction and Zhenwu Decoction+pathway ac-tivator groups showed further reductions in the above parameters and a greater increase in IκBα mRNA(P<0.05).Compared with the Zhenwu Decoction group,the Zhenwu Decoction+pathway activator group had sig-nificantly higher levels of blood glucose,24-hour urinary protein,SCR,BUN,MCP-1,TNF-α,IL-1β mRNA,collagen deposition area,FN,Col I,Wnt4,p-GSK3β,β-catenin,and α-SMA,and lower IκBα mRNA(P<0.05).Conclusion:Zhenwu Decoction alleviates renal injury,inhibits inflammatory response,improves renal function and renal fibrosis in DN rats,and its mechanism may be related to inhibiting Wnt/β-catenin signaling pathway.
