类风湿性关节炎中破骨细胞乳酸堆积的调控机制研究进展OA
Research progress on the regulation of lactate accumulation in osteoclasts from rheumatoid arthritis
类风湿性关节炎(Rheumatoid arthritis,RA)是一种可累及多关节的慢性系统性自身免疫性疾病,以慢性滑膜炎、血管翳形成为核心病理特征,最终造成关节软骨与骨炎症性破坏.该病不仅可导致患者关节畸形、残疾,还会引发心血管疾病等关节外并发症,显著增加死亡率并严重影响患者生活质量.目前RA的传统治疗策略多依赖于非甾体类抗炎药物控制滑膜组织炎症,但往往忽视了破骨细胞在疾病进展中的核心作用及治疗潜力.破骨细胞是介导RA关节骨侵蚀与破坏的核心效应细胞,其代谢重编程对维持成骨-破骨平衡及调控骨吸收过程至关重要.本文重点关注病理状态下破骨细胞中能量代谢向无氧氧化转变这一核心特征,梳理了破骨细胞中无氧氧化代谢产物乳酸堆积的多重成因,阐述了乳酸堆积对破骨细胞分化和骨吸收功能的调控机制,并总结了靶向乳酸代谢在RA治疗中的独特优势与应用潜力,旨在为开发新的临床疗法提供理论依据.
Rheumatoid arthritis(RA)is a chronic,systemic autoimmune disease that can affect multiple joints.Its core pathological features include chronic synovitis and pannus formation,ultimately leading to inflammatory destruction of articular cartilage and bone.The disease not only causes joint deformity and disability,but also triggers extra-articular complications such as cardiovascular diseases,significantly increasing mortality and severely impairing patients'quality of life.Conventional RA treatment strategies largely rely on non-steroidal anti-inflammatory drugs to control synovial tissue inflammation,often overlooking the pivotal role of osteoclasts in disease progression and their therapeutic potential.Osteoclasts serve as key effector cells mediating joint bone erosion and destruction in RA,and their metabolic reprogramming is crucial for maintaining osteoblast-osteoclast balance and regulating bone resorption.This review focuses on the shift of energy metabolism toward anaerobic oxidation in osteoclasts under pathological conditions.It systematically outlines the multiple causes of lactate accumulation resulting from anaerobic oxidation in osteoclasts,elaborates on the regulatory effects of lactate accumulation on osteoclast differentiation and bone resorption function,and further summarizes the unique advantages and application potential of targeting lactate metabolism in RA treatment.The aim is to provide a theoretical basis for the development of novel clinical therapies.
