Diagnostic value of the HALP score in radiation enteritis

目的 探讨血红蛋白、白蛋白、淋巴细胞及血小板(hemoglobin,albumin,lymphocyte,and platelet,HALP)评分与放射性肠炎患者临床病理特征的关系,并采用动物实验评估HALP评分对放射性肠炎的诊断价值.方法 回顾性分析江南大学附属医院2021年1月至2023年12月收治的122例宫颈癌患者的临床资料,分析HALP评分与放射性肠炎发生的临床病理特征的关系.此外,将12只雄性SPF级、6~8周龄C57BL/6J小鼠,随机分为2组:对照组(Control组)和放射性肠炎模型组(RE组).RE组小鼠进行10 Gy的全腹部照射以建立放射性肠炎模型,实验终点时,采集血液及结肠组织,分别进行HE染色、ELISA、血常规及生化检测,以验证HALP评分对放射性肠炎的诊断价值.结果 HALP评分对宫颈癌接受放射治疗后发生放射性肠炎风险预测的受试者工作特征(receiver operating characteristic,ROC)曲线下面积为0.879,HALP评分的最佳截断值为19.96,其敏感度为80.65%,特异度为85.00%.动物实验显示,HALP评分与炎症因子白细胞介素(IL)-6、肿瘤坏死因子-α(TNF-α)、转化生长因子-β(TGF-β)水平呈线性负相关,与IL-10水平呈正相关,与血管内皮生长因子(VEGF)和血管紧张素(Ang)Ⅰ水平的变化呈线性负相关.结论 HALP评分降低与放射性肠炎患者的炎症反应密切相关,评分越低,炎症反应可能越重,HALP评分对放射性肠炎具有潜在的诊断价值.

Objective To investigate the association between the hemoglobin,albumin,lymphocyte,and platelet(HALP)score and clinicopathological characteristics of radiation enteritis(RE),and to evaluate its diagnostic value u-sing both clinical data and an experimental mouse model.Methods A retrospective analysis was conducted on 122 pa-tients with cervical cancer treated at the Affiliated Hospital of Jiangnan University between January 2021 and December 2023.The relationship between HALP score and the occurrence of radiation enteritis was assessed.In addition,12 male SPF-grade C57BL/6J mice(6-8 weeks old)were randomly assigned to a control group and a radiation enteritis(RE)group.The RE model was established by whole-abdomen irradiation at 10 Gy.At the experimental endpoint,blood and colon tissues were collected for histopathological examination(H&E staining),enzyme-linked immunosorbent assay(ELISA),complete blood count,and biochemical analyses to validate the diagnostic value of the HALP score.Results The area under the receiver operating characteristic(ROC)curve of the HALP score for predicting radiation enteritis was 0.879.The optimal cutoff value was 19.96,with a sensitivity of 80.65%and specificity of 85.00%.In the animal mod-el,the HALP score was negatively correlated with pro-inflammatory cytokines[interleukin(IL)-6,tumor necrosis fac-tor-α(TNF-α),and transforming growth factor-β(TGF-β)]and positively correlated with the anti-inflammatory cytokine IL-10.Additionally,HALP showed negative correlations with vascular endothelial growth factor(VEGF)and angiotensin(Ang Ⅰ)levels.Conclusion A lower HALP score is closely associated with enhanced inflammatory responses in radiation enteritis.The HALP score demonstrates potential as a non-invasive diagnostic biomarker for radiation enteritis.
