从"精血同源"探讨青年男性冠心病与弱精症共病的中医病机及治疗思路OA

Exploration on traditional Chinese medicine pathogenesis and treatment of coronary heart disease comorbid with asthenozoospermia in young men based on the theory of homology of essence and blood

45岁以下青年男性冠心病与弱精症共病率显著上升,西医多将二者分属心血管、生殖系统论治,中医"整体观念"则为此类跨系统病症提供了独特思路.以"精血同源"为核心剖析冠心病共病弱精症病机为肾精亏虚动摇精血根基,脾失健运致精血乏源且生痰浊,肝失疏泄引发气机不畅、精血瘀滞.肾脾肝功能失调,终致"心肾不交、水火不济",形成"精血亏虚为本、痰瘀互结为标",临床应以"固肾填精、滋养精血为本,兼以化痰祛瘀、调和脏腑为标"为总治则,针对相关证型分证论治.

The comorbidity rate of coronary heart disease and asthenozoospermia in young men aged≤45 years old has increased significantly.Western medicine mostly treats the two separately as cardiovascular and reproductive systems,while the"holistic concept"of traditional Chinese medicine provides a unique idea for such cross-system diseases.This article analyzes the mechanisms underlying comorbidity,centered on the concept"homology of essence and blood",namely,kidney essence deficiency undermining the foundation of essence and blood,spleen dysfunction leading to insufficient production of essence and blood and generating phlegm turbidity,and liver stagnation causing qi stagnation and essence-blood stasis.The imbalance of kidney,spleen and liver function eventually leads to"disharmony between heart and kidney,imbalancce between water and fire",forming a pattern of"deficiency of essence and blood as the root cause,and phlegm and blood stasis intemingling as the manifestation".Based on this,the general treatment principle of"consolidating kidney and replenishing essence,nourishing essence and blood as the foundation,while also focusing on resolving phlegm and removing blood stasis,harmonizing zang-fu organs as the standard"is proposed.For the relevant syndrome patterns,treatment based on syndrome differentiation is implemented.
