目的:探讨三七总皂苷(PNS)对大鼠放射性心脏损伤(RIHD)的保护作用及相关作用机制.方法:选取50只大鼠按数字表法随机分为正常对照组、单纯照射组及PNS小、中、大剂量组,每组各10只.除正常对照组外,其余各组均予以放射线心脏照射(20 Gy);PNS小、中、大剂量组分别予以10 mg/kg、30 mg/kg、60 mg/kg PNS灌胃处理,其余各组则予以等量生理盐水灌胃,于放疗当天开始,每天连续给药,直至放疗4周.放疗照射结束后,检测大鼠心肌肌钙蛋白I(cTnI)、肌酸激酶同工酶(CK-MB)、丙二醛(MDA)、超氧化物歧化酶(SOD)、细胞间黏附分子-1(ICAM-1)、肿瘤坏死因子α(TNF-α)、白细胞介素1β(IL-1β)、转化生长因子β1(TGF-β1)水平;HE染色对大鼠心肌组织病理改变进行观察;蛋白质印迹法检测大鼠心肌组织TGF-β1、Smad3蛋白表达水平.结果:与正常对照组比,单纯照射组大鼠血清cTnI、CK-MB、MDA、ICAM-1、TNF-α、IL-1β、TGF-β1水平上升(P<0.05),血清SOD水平下降(P<0.05),且心肌组织病理评分增高(P<0.05).相比于单纯照射组,PNS各剂量组大鼠血清cTnI、CK-MB、MDA、ICAM-1、TNF-α、IL-1β、TGF-β1水平下降(P<0.05),血清SOD水平上升(P<0.05),且心肌组织病理评分降低(P<0.05),这些指标的改变在小、中、大剂量组呈剂量依赖性(P<0.05).结论:PNS对大鼠RIHD具有保护作用,其机制可能与调控氧化应激、减轻炎症及抑制TGF-β1/Smad3信号通路有关.

Objective:To investigate the protective effect of Panax Notoginseng Saponins(PNS)on radiation-induced heart disease(RIHD)in rats and explore its underlying mechanism.Methods:A total of 50 rats were randomly divided into 5 groups using a random number table:normal control group,radiation-only group,and low-,medium-,and high-dose PNS intervention groups,with 10 rats in each group.Except for the normal control group,rats in the other four groups received a single 20 Gy cardiac irradiation.Rats in the low-,medium-,and high-dose PNS groups were intragastrically administered with 10 mg/kg,30 mg/kg,and 60 mg/kg PNS,respectively,while those in the normal control and radiation-only groups received an equal volume of normal saline.The administration was initiated on the day of irradiation and continued daily for 4 weeks after radiotherapy.After the irradiation,serum levels of cardiac troponin I(cTnI),creatine kinase isoenzyme MB(CK-MB),malondialdehyde(MDA),superoxide dismutase(SOD),intercellular adhesion molecule-1(ICAM-1),tumor necrosis factor-α(TNF-α),interleukin-1β(IL-1β),and transforming growth factor-β1(TGF-β1)were detected.Pathological changes of myocardial tissues were observed using hematoxylin-eosin(HE)staining.Western blot was performed to determine the protein expression levels of TGF-β1 and Smad3 in myocardial tissues.Results:Compared with the normal control group,the radiation-only group showed increased serum levels of cTnI,CK-MB,MDA,ICAM-1,TNF-α,IL-1β,and TGF-β1(P<0.05),decreased serum SOD level(P<0.05),and elevated myocardial pathological score(P<0.05).Compared with the radiation-only group,all PNS intervention groups exhibited reduced serum levels of cTnI,CK-MB,MDA,ICAM-1,TNF-α,IL-1β,and TGF-β1(P<0.05),increased serum SOD level(P<0.05),and decreased myocardial pathological score(P<0.05).The changes of these indexes were dose-dependent in the low,medium and high dose groups(P<0.05).Conclusion:PNS exerts a protective effect against RIHD in rats,and its mechanism may be associated with regulating oxidative stress,alleviating inflammatory response,and inhibiting the TGF-β1/Smad3 signaling pathway.
