黄厚止泻滴丸治疗腹泻型肠易激综合征临床研究OA
Clinical Research on Huanghou Zhixie Dripping Pills in Treating Diarrhea-Predominant Irritable Bowel Syndrome
目的 观察黄厚止泻滴丸治疗腹泻型肠易激综合征(IBS-D)的疗效及机制.方法 采用随机对照试验设计纳入72例IBS-D患者,按随机数字表法分为试验组(口服黄厚止泻滴丸)和对照组(口服双歧杆菌四联活菌片),每组36例.另设20名健康志愿者为健康对照组.评估指标包括临床疗效、肠道菌群结构(16S rRNA测序)、胆汁酸谱(靶向代谢组学)及血清炎症因子与肠屏障功能标志物(ELISA).结果 治疗4周后,试验组总有效率(97.2%)高于对照组(45.7%,x2=20.79,P<0.01),且在改善腹痛评分(NRS)、Bristol粪便性状评分、IBS严重程度评分(IBS-SSS)方面均优于对照组(P<0.05).分析表明,黄厚止泻滴丸可调节肠道菌群结构,表现为拟杆菌门(Bacteroidota)和普雷沃菌属(Prevotella_9)丰度升高,厚壁菌门(Firmicutes)及促炎菌属(如Escherichia_Shigella)丰度降低;降低次级胆汁酸3-脱氢胆酸(3-DHCA)水平;并有效降低血清促炎因子(TNF-α、IL-6)、神经递质5-羟色胺(5-HT)及肠屏障损伤标志物(血清连蛋白、钙卫蛋白)水平(P<0.01).结论 黄厚止泻滴丸总有效率及症状改善效果均优于双歧杆菌四联活菌片,其作用可能与协同调控肠道菌群-胆汁酸代谢-免疫屏障网络相关.
Objective To observe the effectiveness and mechanism of Huanghou Zhixie Dripping Pills(HZDP)in treating diarrhea-predominant irritable bowel syndrome(IBS-D).Methods A randomized controlled trial design was adopted to include 72 patients with IBS-D.Patients were randomly assigned to the treatment group(given HZDP for oral use)and the control group[given Combined Bifidobacterium,Lactobacillus,Enterococcus and Bacillus cereus Tablets,(live)for oral use],with 36 cases in each group.Additionally,20 healthy volunteers were enrolled as the healthy control group.Evaluation indicators included clinical efficacy,intestinal flora structure(assessed via 16S rRNA sequencing),bile acid profiles(analyzed by targeted metabolomics),serum inflammatory factors and markers of intestinal barrier function(measured by ELISA).Results After 4 weeks of treatment,the total effective rate of the treatment group(97.2%)was significantly higher than that of the control group(45.7%,x2=20.79,P<0.01).Moreover,it also showed superior improvements in the Numerical Rating Scale(NRS)for abdominal pain,Bristol Stool Form Scale,and IBS Symptom Severity Scale(IBS-SSS)compared to the control group(P<0.05).The analysis showed that HZDP could regulate the intestinal flora structure,manifested as increased in abundance of Bacteroidota and Prevotella_9,and decreased abundance of Firmicutes and pro-inflammatory genera(e.g.,Escherichia_Shigella),Additionally,it markedly reduced the concentration of 3-dehydrocholic acid(3-DHCA),a secondary bile acid,and significantly lowered the levels of serum pro-inflammatory factors(TNF-α,IL-6),5-hydroxytryptamine(5-HT),and markers of intestinal barrier damage(Zonulin and Calprotectin)(P<0.01).Conclusions The total effective rate and symptom improvement effect of HZDP were both superior to those of Combined Bifidobacterium,Lactobacillus,Enterococcus and Bacillus cereus Tablets(live).Its therapeutic effect may be related to the coordinated regulation of the intestinal flora-bile acid metabolism-immune barrier network.
