目的:评估布地奈德福莫特罗、噻托溴铵联合疗法对急性加重期慢性阻塞性肺疾病(AECOPD)患者的治疗效果.方法:选取 2023 年 3 月—2025 年 5 月就诊于浦城县医院的 102 例 AECOPD 患者,本研究为前瞻性随机对照研究,所有患者经随机数表法均分两组,其中对照组(51 例)采用布地奈德福莫特罗治疗,研究组(51 例)在对照组基础上加用噻托溴铵联合干预.比较两组临床疗效、通气功能、炎症因子、血管内皮功能、不良反应.结果:研究组总有效率高于对照组;治疗后,研究组第 1 秒用力呼气容积(FEV1)、最大中期呼气流量(MMEF)、每分钟最大通气量(MVV)及一氧化氮(NO)水平均高于对照组;同时,研究组内皮素-1(ET-1)、白三烯 B4(LTB4)、肿瘤坏死因子 α(TNF-α)及 C 反应蛋白(CRP)水平均低于对照组,上述各项指标差异均具有统计学意义(P<0.05).两组不良反应比较,差异无统计学意义(P>0.05).结论:在 AECOPD 的治疗中,布地奈德福莫特罗与噻托溴铵联用疗效确切,不仅能增强肺通气功能,还可抑制炎症反应、改善血管内皮功能,兼具良好安全性.

Objective:To evaluate the therapeutic effectiveness of a combined therapeutic strategy involving budesonide formoterol and tiotropium bromide in patients with acute exacerbation of chronic obstructive pulmonary disease(AECOPD).Method:This prospective randomized controlled research recruited 102 AECOPD patients who received treatment at Pucheng County Hospital between March 2023 and May 2025.All the patients were randomly divided into two groups by using the random number table method.The control group(51 cases)were treated with budesonide formoterol alone,while the research group(51 cases)were given combined therapy with budesonide formoterol plus tiotropium bromide.Clinical therapeutic effects,respiratory function parameters,inflammatory biomarkers,endothelial function,and adverse events were evaluated and compared between the two groups.Result:Compared with the control group,the total effective rate of the research group was remarkably higher.After the treatment was completed,forced expiratory volume in the first second(FEV1),maximal mid-expiratory flow(MMEF),minute maximum ventilation(MVV),and nitric oxide(NO)in the research group were all higher than those in the control group.In addition,the levels of endothelin-1(ET-1),leukotriene B4(LTB4),tumor necrosis factor alpha(TNF-α),and C-reactive protein(CRP)in the research group were lower than those in the control group,and all these indicator differences were all statistically significant(P<0.05).The occurrence of adverse reactions was compared between the two groups,and the difference was not statistically significant(P>0.05).Conclusion:The combination therapy of budesonide,formoterol,and tiotropium bromide provides a distinct therapeutic benefit in the management of AECOPD.It not only enhances pulmonary ventilatory function and inhibits inflammatory reactions but also improves endothelial function,accompanied by a favorable safety profile.
