正清风痛宁关节腔注射联合硫酸氨基葡萄糖治疗对膝关节骨性关节炎患者膝关节功能和血清IL-6、TNF-α水平及骨代谢标志物的作用观察OA
Observation of the Effect of Zhengqingfeng Tongning Joint Cavity Injection Combined with Sulfated Glucosamine on Knee Joint Function,Serum IL-6,TNF-α Levels,and Bone Metabolism Markers in Patients With Knee Osteoarthritis
目的:探讨正清风痛宁关节腔注射联合硫酸氨基葡萄糖治疗对膝关节骨性关节炎(KOA)患者膝关节功能和血清白介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)水平及骨代谢标志物的作用.方法:选取 2024 年 1 月—2025 年 3 月甘肃省第二人民医院收治的 96 例KOA 患者,按照抽签法随机分为GS 组和GS+ZT组,每组各 48 例.两组均予以常规基础治疗,GS 组在常规治疗基础上给予硫酸氨基葡萄糖治疗,GS+ZT组在GS组基础上给予正清风痛宁关节腔注射治疗.比较两组治疗前后膝关节功能[Lysholm评分、西安大略和麦克马斯特大学骨关节炎指数(WOMAC)]、膝关节活动度(ROM)、血清炎症因子[IL-6、白介素-17(IL-17)、TNF-α]、骨代谢标志物[软骨寡聚基质蛋白(COMP)、基质金属蛋白酶-13(MMP-13)、骨钙素(BGP)]变化情况及用药期间不良反应发生情况.结果:治疗后,GS+ZT组和GS 组的 Lysholm 评分、ROM、BGP 均较治疗前明显上升,且GS+ZT组的上述指标比同期 GS 组更高,差异有统计学意义(P<0.05);GS+ZT 组和 GS 组的 WOMAC 评分,IL-6、IL-17、TNF-α、COMP 和 MMP-13水平均较治疗前下降,GS+ZT 组的上述指标低于同期 GS 组,差异均有统计学意义(P<0.05);治疗阶段,两组均未报告明显的不良反应,差异无统计学意义(P>0.05).结论:采用正清风痛宁关节腔注射联合硫酸氨基葡萄糖治疗 KOA,可显著改善患者膝关节功能,减轻炎症并调节骨代谢,且安全性良好,适于临床应用.
Objective:To investigate the effects of joint cavity injection of Zhengqingfeng Tongning combined with glucosamine sulfate on knee joint function,serum interleukin-6(IL-6),tumor necvosis factor-α(TNF-α)levels,and bone metabolism markers in patients with knee osteoarthritis(KOA).Method:96 patients with knee osteoarthritis(KOA)admitted to the Second People's Hospital of Gansu Province from January 2024 to March 2025 were randomly divided into the Glucosamine Sulfate group and the Glucosamine Sulfate+Zhengqingfeng Tongning group by drawing lots,with 48 cases in each group.Both groups of patients received routine basic treatment.The GS group received treatment with glucosamine sulfate in addition to routine treatment,while the GS+ZT group received joint cavity injection of Zhengqingfeng Tongning in addition to routine treatment.The changes in knee joint function[Lysholm score,Western Ontario and McMaster University Osteoarthritis Index(WOMAC)],knee joint range of motion(ROM),serum inflammatory factors[IL-6,interleukin-17(IL-17),TNF-α],bone metabolism markers[cartilage oligomeric matrix protein(COMP),matrix metalloproteinase-13(MMP-13),osteocalcin(BGP)],and adverse reactions during medication between two groups were compared before and after treatment.Result:After treatment,the Lysholm score,ROM,and BGP of the GS+ZT group and GS group significantly increased compared to before treatment,and the above indicators of the GS+ZT group were higher than those of the GS group at the same time,with statistically significant differences(P<0.05).The WOMAC scores,as well as the levels of IL-6,IL-17,TNF-α,COMP,and MMP-13,decreased in both the GS+ZT group and the GS group compared to before treatment.The aforementioned indicators in the GS+ZT group were lower than those in the GS group at the same time,with statistically significant differences(P<0.05).During the treatment phase,no significant adverse reactions were reported in either group,and the difference was not statistically significant(P>0.05).Conclusion:The combination of Zhengqingfeng Tongning intra-articular injection and sulfated glucosamine can significantly improve knee joint function,reduce inflammation,and regulate bone metabolism in patients with knee osteoarthritis,with good safety and suitable for clinical application.
