一类洞纳米树脂充填修复的临床观察OA
Clinical Observation of Nanocomposite Resin Filling and Restoration for Class Ⅰ Cavity
目的:探讨纳米树脂充填治疗一类洞龋齿患者的修复效果及咀嚼功能恢复情况,为基层医院一类洞龋齿修复方案选择提供依据.方法:回顾性分析 2023 年 10 月—2025 年 10 月在莆田市城厢区医院接受纳米树脂充填治疗的 301 例龋齿患者(301 颗患牙)的临床资料,其中女性 163 例,男性 138 例.所有患者均采用纳米复合树脂进行充填治疗,分别于治疗前及治疗后 6、12、24 个月采用改良 USPHS 标准评价修复体边缘密合性、解剖形态、表面质地等指标,采用咀嚼效率测定法评估咀嚼功能,采用视觉模拟评分法(VAS)评价牙本质敏感程度,记录继发龋、充填体脱落等并发症发生情况.结果:不同性别患者的年龄、龋坏部位及程度分布比较,差异无统计学意义(P>0.05).治疗后 6、12、24 个月,修复体边缘密合性、解剖形态、表面质地优良率均保持在92%以上,各时间点间比较,差异无统计学意义(P>0.05);治疗后 24 个月,咀嚼效率高于治疗前,牙本质敏感 VAS 评分低于治疗前,差异有统计学意义(P<0.05).咬合功能评价显示,治疗后 24 个月咬合功能恢复优良率达 98.67%.随访 24 个月期间,共发生继发龋 6 例(1.99%),充填体微渗漏 9 例(2.99%),无充填体脱落或折裂病例.结论:纳米树脂充填治疗一类洞龋齿可维持良好的修复体稳定性及咀嚼功能,并发症发生率低,操作简便且无需特殊仪器,是基层医院治疗一类洞龋齿的理想方案.
Objective:To explore the restoration effect and chewing function recovery of patients with Class I dental caries treated with nano resin filling,and provide a basis for the selection of restoration plans for Class I dental caries in primary hospitals.Method:The clinical data of 301 patients with dental caries(301 teeth)who received nano-resin filling treatment in the Department of Stomatology,Chengxiang District Hospital of Putian City from October 2023 to October 2025 were retrospectively analyzed,including 163 females and 138 males.The marginal fitness,anatomical morphology,and surface texture of the restoration were evaluated by modified USPHS criteria before treatment and 6,12,and 24 months after treatment.The masticatory function was evaluated by masticatory efficiency,and the dentin sensitivity was evaluated by visual analogue scale(VAS).The complications such as secondary caries and restoration loss were recorded.Result:There was no significant difference in age,location and degree of caries between male and female patients(P>0.05).At 6,12,and 24 months after treatment,the excellent and good rates of marginal fitness,anatomical morphology,and surface texture of the prosthesis were all above 92%,and there was no significant difference between different time points(P>0.05).At 24 months after treatment,the masticatory efficiency was higher than that before treatment,and the dentin hypersensitivity VAS score was lower than that before treatment,with statistically significant differences(P<0.05).The occlusal function evaluation showed that the excellent and good rate of occlusal function recovery was 98.67%at 24 months after treatment.During the follow-up period of 24 months,there were 6 cases(1.99%)of secondary caries,9 cases(2.99%)of microleakage of the filling body,and no cases of breakage or fracture of the filling body.Conclusion:Nano resin filling therapy can maintain good restoration stability and chewing function in the treatment of Class I dental caries,with a low incidence of complications,simple operation,and no need for special instruments.It is an ideal solution for treating Class I dental caries in primary hospitals.
