目的:探讨将重组人TNK组织型纤溶酶原激活剂(rhTNK-tPA)与依达拉奉右莰醇联合用于脑梗死的治疗中所产生的效果.方法:选取2023年1月—2025年2月泗洪医院收治的72例脑梗死患者作为研究对象,根据入院时间进行分组,将2023年1月—2024年1月收治的36例患者纳入对照组(采用rhTNK-tPA治疗),将2024年2月—2025年2月收治的36例患者纳入观察组(采用rhTNK-tPA+依达拉奉右莰醇治疗),比较两组神经功能、治疗效果、认知功能、血管内皮功能.结果:观察组治疗7 d、14 d后的神经元特异性烯醇化酶(NSE)、基质金属蛋白酶9(MMP-9)、中枢神经特异性蛋白β(S100β)水平均较对照组低;治疗7 d 后的血清内皮素 1(ET-1)、一氧化氮(NO)水平低于对照组,蒙特利尔认知评估量表(MOCA)各项评分与睾酮(T)水平高于对照组;在治疗总有效率上,观察组高于对照组,上述差异均有统计学意义(P<0.05).结论:在治疗脑梗死过程中,充分发挥 rhTNK-tPA 与依达拉奉右莰醇的联合优势能够让患者的神经功能有更好地恢复,同时改善认知功能与血管内皮功能,提升治疗效果.

Objective:To explore the effect produced by the combination of Recombinant Human TNK Tissue Plasminogen Activator(rhTNK-tPA)and edaravone dexcamphenol in the treatment of cerebral infarction.Method:72 patients with cerebral infarction who were admitted to Sihong Hospital in January 2023 were selected as the research subjects.The time period ended in February 2025.The patients were grouped based on the admission time.Among them,the patients(36 cases)in the period from January 2023 to January 2024 received rhTNK-tPA treatment and were classified as the control group.A total of 36 patients admitted from February 2024 to February 2025 were included in the observation group(treated with rhTNK-tPA+edaravone and dexcamphenol),and the neurological function,therapeutic effect,cognitive function and vascular endothelial function of the two groups were compared.Result:The levels of neuron-specific enolase(NSE),matrix metalloproteinase 9(MMP-9),and central nervous system specific protein β(S100β)in the observation group after 7 days and 14 days of treatment were all lower than those in the control group.After 7 days of treatment,the levels of serum endothelin-1(ET-1)and nitric oxide(NO)were lower than those in the control group,while the scores of each item of the Montreal Cognitive Assessment Scale(MOCA)and the level of testosterone(T)were higher than those in the control group.In terms of the total effective rate of treatment,the observation group was higher than the control group.The above differences were all statistically significant(P<0.05).Conclusion:In the treatment of cerebral infarction,giving full play to the combined advantages of rhTNK-tPA and edaravone dexcamphenol can better restore the neurological function of patients,improve cognitive function and vascular endothelial function at the same time,and enhance the therapeutic effect.
