[目的]基于中医肝主疏泄理论,揭示肝失疏泄、胆汁酸代谢紊乱与乳腺癌发病的内在关联,阐明肝郁致癌的分子生物学基础,为中西医结合防治乳腺癌提供新思路.[方法]系统梳理肝主疏泄的核心内涵,分析胆汁酸代谢紊乱对乳腺癌的影响,整合中医学理论与现代医学研究,构建"肝失疏泄-胆汁酸代谢紊乱-乳腺癌"的理论逻辑,并针对该机制总结中医药防治依据.[结果]肝失疏泄作为核心病机,通过气滞、痰瘀等病理因素影响乳腺癌全过程.现代研究发现,胆汁酸代谢紊乱与乳腺癌密切相关,生理状态下特定胆汁酸成分通过激活武田G蛋白偶联受体 5(Takeda G protein-coupled receptor 5,TGR5)/法尼醇X受体(farnesoid X receptor,FXR)发挥抑癌监视作用,病理状态下胆汁酸谱失衡及信号异常为乳腺癌发生创造条件.肝失疏泄通过影响胆汁分泌排泄、干扰肠肝循环及肠道菌群平衡,诱发代谢紊乱,增加乳腺癌风险.疏肝理气类中药可通过调控胆汁酸相关信号通路,纠正代谢紊乱,抑制肿瘤发展.[结论]肝疏泄功能失常诱导的胆汁酸代谢失衡与乳腺癌发病具有关联性,阐明该机制有助于为中西医结合防治乳腺癌提供创新范式,也为乳腺癌防治领域的理论研究与临床实践提供新的方向.

[Objective]To reveal the intrinsic link between impaired liver Qi flow,bile acid metabolism disorders,and breast cancer development,elucidate the molecular biological basis of liver Qi stagnation-induced carcinogenesis,in order to offer new insights for the integrated prevention and treatment of breast cancer through Chinese and western medicine,based on the traditional Chinese medicine(TCM)theory of liver governing catharsis.[Methods]Systematically examining the core concept of liver regulation in promoting Qi flow,analyzing the impact of bile acid metabolism disorders on breast cancer,this research integrates TCM theory with modern medical research,establishes the theoretical logic linking"liver dysfunction in promoting smooth flow-bile acid metabolism disorder-breast cancer",and summarizes the basis for prevention and treatment with TCM based on this mechanism.[Results]Liver dysfunction in regulating Qi flow serves as the core pathogenesis,affecting the entire course of breast cancer through pathological factors such as Qi stagnation and phlegm-stasis.Modern research reveals a close association between bile acid metabolism disorders and breast cancer:under physiological conditions,specific bile acid components exert tumor-suppressing surveillance by activating Takeda G protein-coupled receptor 5(TGR5)/farnesoid X receptor(FXR);conversely,pathological imbalances in bile acid profiles and abnormal signaling create conditions conducive to breast cancer development.Impaired liver Qi regulation disrupts bile secretion and excretion,interferes with enterohepatic circulation,and disturbs gut microbiota balance,thereby inducing metabolic disorders and increasing breast cancer risk.Liver-soothing and Qi-regulating Chinese herbal medicines can correct metabolic dysregulation and inhibit tumor progression by modulating bile acid-related signaling pathways.[Conclusion]Impaired bile acid metabolism induced by abnormal liver detoxification function is associated with breast cancer development.Elucidating this mechanism will help provide an innovative therapeutic paradigm for the prevention and treatment of breast cancer through the integration of Chinese and western medicine,and also offer new directions for theoretical research and clinical practice in breast cancer prevention and treatment.
