[目的]运用数据挖掘技术,系统总结国家级名老中医连建伟教授应用桑菊饮治疗温病的用药规律,并阐释其学术思想.[方法]收集2003年8月至2013年7月连教授门诊的534例桑菊饮治疗外感温病的医案,并筛选整理病例数据;对原方与高频加味药的使用频次及高频加味药的性味归经进行统计分析;采用Jaccard系数与Ward法对高频加味药进行聚类分析;运用Apriori算法挖掘症状-药物关联,并分析当归的定向补充;结合Phi系数、多因素Logistic回归和随机森林模型验证关键用药指征及辨证梯度路径.[结果]桑菊饮治疗外感温病的用药频次中,辛凉解表类药物使用较多,且高频加味药物符合温病治法的常见药性.聚类分析显示,药物之间形成稳定的配伍模式.症状与药物关联规则分析揭示,咽痛、咳嗽等症状与桑菊饮加味药物的应用关系密切.多模型验证揭示,"舌有瘀点/瘀斑"是当归使用的核心指征(OR=25.97,95%CI:9.30~71.50);当风邪征象与舌瘀共现时,当归使用置信度达90.91%.[结论]连教授运用桑菊饮的核心规律在于以高度稳定的原方结构恪守吴鞠通"辛凉轻剂"法度,通过规律性药群应对兼夹病机;其当归应用策略严格遵循"舌瘀"核心指征,并随辨证要素叠加而动态调整.该结果为名老中医隐性经验的数据驱动式挖掘提供了研究范例.

[Objective]To investigate Professor LIAN Jianwei's medication patterns in the treatment of warm disease(Wenbing)by using Sangju Yin,and to elucidate his academic thought through data mining.[Methods]A total of 534 medical records involving Sangju Yin prescribed by Professor LIAN from August 2003 to July 2013 were collected.The case data were screened and organized.Frequency analysis was conducted to evaluate the usage frequency of the original formula and high-frequency added herbs,along with their properties(taste and meridian entry).Hierarchical cluster analysis(Ward's method;Jaccard coefficient)was used to identify functional clusters of high-frequency added herbs.Apriori algorithm was applied to explore symptom-herb associations,with a focused analysis of Danggui-related rules.Phi coefficients,multivariable logistic regression[odds ratio(OR)],and random-forest feature importance were used to validate key medication indications and the diagnostic progression pathways.[Results]Frequency analysis indicated that,in the treatment of warm disease with Sangju Yin,acrid-cool herbs were frequently used,and the high-frequency added herbs followed the common medicinal properties in warm disease treatment.Cluster analysis revealed stable herb-pairing patterns.Symptom-herb association rule analysis showed that symptoms such as sore throat and cough were closely related to the use of high-frequency added herbs in Sangju Yin.Multi-model validation revealed that"tongue petechiae/ecchymosis"was a core indication for using Danggui(OR=25.97,95%CI:9.30-71.50);when wind-cold symptoms co-occurred with tongue stasis,the confidence for using Danggui reached 90.91%.[Conclusion]Professor LIAN's clinical use of Sangju Yin was characterized by adherence to a stable base-formula structure in line with WU Jutong's principle of"acrid-cool,light medicinals",while employing patterned herb clusters to address concomitant pathogenetic factors.The use of Danggui was primarily guided by tongue-stasis indicators and dynamically adjusted according to the accumulation of syndrome elements.This results provides a data-driven example for mining the tacit clinical experience of renowned senior traditional Chinese medical practitioners.
