针刺治疗类风湿关节炎免疫调节机制的研究进展OA
Research Progress on Immune Regulation Mechanisms of Acupuncture in Treatment of RA
类风湿关节炎作为一种慢性自身免疫性疾病,发病核心在于免疫系统对关节组织的异常攻击.针刺作为传统中医疗法,能通过调节T细胞亚群平衡、抑制炎症因子释放及调控神经-内分泌-免疫网络进而发挥治疗作用.近年来随着对针刺机制研究的深入,其在类风湿关节炎中的免疫调节作用日益明确,能通过多靶点、多层次的免疫调节机制改善患者症状.然而,现有研究多局限于动物模型与小样本临床研究,机制探讨亦较为表浅,缺乏多中心大样本的高质量证据支持.基于此,本研究从针刺免疫调节的分子机制及信号通路研究进展入手,系统梳理针刺干预下JAK/STAT、NF-κB与MAPK等关键炎症通路的调控作用,进一步揭示其多靶点、整体性调节优势,为针刺治疗类风湿关节炎的科学内涵提供现代医学支撑.
Rheumatoid arthritis(RA),as a chronic autoimmune disease,is characterized by aberrant immune-mediated attacks on joint tissues.Acupuncture,as a traditional therapeutic modality in Chinese medicine,exerts therapeutic effects by regulating the balance of T-cell subsets,inhibiting the release of inflammatory cytokines,and modulating the neuroendocrine-immune network.In recent years,with the deepening of mechanistic studies on acupuncture,its immunoregulatory effects in RA have become increasingly evident.Acupuncture has been shown to alleviate symptoms in patients with RA through multi-target and multi-level immune regulatory mechanisms.However,existing studies are still largely limited to animal models and small-sample clinical observations,and mechanistic investigations remain relatively superficial,lacking support from high-quality evidence derived from multicenter,large-sample studies.Based on this,this study focuses on recent advances in the molecular mechanisms and signaling pathways underlying the immunoregulatory effects of acupuncture.It systematically summarizes the regulatory effects of acupuncture on key inflammatory pathways,including JAK/STAT,NF-κB,and MAPK,thereby further elucidating its advantages in multi-target and holistic regulation and providing a modern medical interpretation of the scientific basis for acupuncture in treatment of RA.
