内热针联合桂枝芍药知母汤治疗强直性脊柱炎随机对照研究OA
Randomized Controlled Trial of Neire Acupuncture Combined with Guizhi Shaoyao Zhimu Decoction in Treatment of AS
目的:观察内热针联合桂枝芍药知母汤治疗强直性脊柱炎(AS)的临床疗效.方法:将90例AS患者按照随机数字表法随机分为对照组、治疗1组及治疗2组,每组30例.对3组患者均予基础治疗,对照组予塞来昔布胶囊口服,治疗1组予塞来昔布胶囊口服+桂枝芍药知母汤口服,治疗2组予塞来昔布胶囊口服+内热针+桂枝芍药知母汤口服,3组均以4周为1个疗程.治疗1个疗程后比较3组的临床疗效及治疗前后ESR、CRP、IL-6、腰背疼痛VAS、晨僵程度VAS、Schober评分、Keitel功能试验及中医症候积分.结果:对照组、治疗1组及治疗2组的总有效率分别为:56.67％(17/30)、70.00％(21/30)和96.67％(29/30),治疗1组、治疗2组的总有效率明显高于对照组,治疗2组的总有效率明显高于治疗1组,3组组间比较,差异具有统计学意义(P＜0.05);治疗后3组ESR、CRP、IL-6、腰背疼痛VAS、晨僵程度VAS、Schober评分、Keitel功能试验及中医症候积分均较同组治疗前改善,差异具有统计学意义(P＜0.05),且治疗1组、治疗2组对各观察指标的改善效果明显优于对照组,差异具有统计学意义(P＜0.05),治疗2组对各观察指标的改善效果明显优于治疗1组,差异具有统计学意义(P＜0.05).结论:内热针联合桂枝芍药知母汤治疗强直性脊柱炎既能改善患者的脊柱关节活动范围,缓解临床症状,又能降低ESR、CRP及IL-6,改善炎症水平,标本同治,疗效显著,且运用安全.
Objective:To observe the clinical efficacy of Neire acupuncture(acupuncture technique for warming the deep tissue of the lesions)combined with Guizhi Shaoyao Zhimu Decoction in treatment of ankylosing spondylitis(AS).Methods:A total of 90 patients with AS were randomly divided into the control group,the treatment group 1,and the treatment group 2,with 30 patients in each group.All patients received basic treatment.The control group received Celecoxib Capsule;on which basis,the treatment group 1 was treated with Guizhi Shaoyao Zhimu Decoction orally,and the treatment group 2 was treated with Nere acupuncture and Guizhi Shaoyao Zhimu Decoction.Each group was treated for one 4-week course.After one course,the clinical efficacy and changes in ESR,CRP,IL-6,and the scores of back pain VAS,morning stiffness VAS,Schober test,Keitel functional test,and TCM symptoms were compared among the three groups.Results:The total effective rate was 56.67％(17/30)in the the control group,70.00％(21/30)the in the treatment group 1,and 96.67％(29/30)in the treatment group 2.Both treatment groups showed significantly higher total effective rates compared with the control group,and the treatment group 2 was significantly higher than the treatment group 1(P＜0.05).After the treatment,all three groups showed significant improvements in ESR,CRP,IL-6,and the scores of back pain VAS,morning stiffness VAS,Schober test,Keitel functional test,and TCM symptoms compared with their respective baseline values(P＜0.05);the improvements were more significant in the treatment group 1 and the treatment group 2 compared with the control group(P＜0.05),and the treatment group 2 demonstrated significantly superior improvements compared with the treatment group 1(P＜0.05).Conclusion:Neire acupuncture combined with Guizhi Shaoyao Zhimu Decoction not only improves spinal joint range of motion and alleviates clinical symptoms but also reduces ESR,CRP,and IL-6,thereby ameliorating inflammatory status in treatment of AS.This approach demonstrates significant efficacy with simultaneous management of symptoms and underlying pathology,and is safe in application.
