Metabolomics study on anti-inflammatory effects of Guanjie Zhentong Babu plaster in collagen-induced arthritis rats

目的:探讨关节镇痛巴布膏对胶原诱导性关节炎(CIA)大鼠的抗炎作用及其潜在机制.方法:大鼠随机分为正常对照组和实验组;实验组采用牛Ⅱ型胶原乳剂免疫诱导建立CIA大鼠模型后,分为模型寒证组、模型热证组、大剂量寒证组、大剂量热证组、小剂量寒证组、阳性对照寒证组、阳性对照热证组、空白巴布膏寒证组,每组8只.分别于给药后第0、4、7、14天等时间点观察大鼠足部关节的关节炎指数(AI)评分,并采用足趾容积测量仪评估大鼠后足关节的肿胀程度.药效学评价结束后处死大鼠,采用苏木精-伊红染色观察大鼠踝关节组织病理学改变;应用超高效液相色谱-线性离子阱-静电场轨道阱质谱联用技术分析大鼠血清代谢物谱,结合代谢通路富集分析探讨药物干预的潜在机制,并通过免疫组织化学法及酶联免疫吸附试验检测肿瘤坏死因子α(TNF-α)、IL-17、IL-10的变化.结果:与模型寒证组比较,大剂量寒证组在第14天时AI评分明显改善(P<0.05),后足关节肿胀程度显著减小(P<0.01),踝关节组织病理学损伤明显减轻;而大剂量热证组与模型热证组相比各项指标无明显变化.代谢组学分析发现,模型寒证组精氨酸生物合成和嘌呤代谢通路发生明显紊乱,与模型寒证组比较,大剂量寒证组L-瓜氨酸含量显著升高、尿酸和黄嘌呤含量显著减少(均P<0.05).免疫组织化学法及酶联免疫吸附试验结果显示,与模型寒证组比较,大剂量寒证组踝关节滑膜组织和血清中TNF-α、IL-17水平明显降低(均P<0.05),而IL-10水平明显升高(均P<0.01).结论:关节镇痛巴布膏可通过调节精氨酸和嘌呤代谢相关通路,重建炎症平衡,进而改善CIA寒证大鼠的关节损伤.

Objective:To investigate the anti-inflammatory effects of Guanjie Zhentong Babu plaster on collagen-induced arthritis(CIA)rats and its potential mechanisms.Methods:Rats were randomly divided into normal control group and experimental group.The experimental group was immunized with bovine type Ⅱ collagen emulsion to establish the CIA rat model,and then subdivided into the following groups:cold syndrome model group,heat syndrome model group,high-dose cold syndrome group,high-dose heat syndrome group,low-dose cold syndrome group,positive control cold syndrome group,positive control heat syndrome group,and blank plaster cold syndrome group,with eight rats in each group.Arthritis index(AI)scores of paw joints were evaluated on day 0,day 4,day 7,and day 14 after drug administration.A plethysmometer was used to assess hind paw joint swelling.After completion of the pharmacodynamic evaluation,the rats were sacrificed,and hematoxylin and eosin staining was performed to observe histopathological changes in ankle joint tissues.Serum metabolite profiles were analyzed using ultra performance liquid chromatography coupled with linear ion trap quadrupole-Orbitrap mass spectrometry,and metabolic pathway enrichment analysis was conducted to explore the potential mechanisms of drug intervention.In addition,immunohistochemistry and enzyme-linked immunosorbent assay were performed to investigate the levels of tumor necrosis factor-α(TNF-α),IL-17,and IL-10.Results:Compared with the cold syndrome model group,the high-dose cold syndrome group showed a significant improvement in AI scores on day 14(P<0.05)and a significant reduction in hind paw joint swelling(P<0.01),accompanied by markedly alleviated histopathological damage in the ankle joint.In contrast,no significant changes were observed in the high-dose heat syndrome group compared with the heat syndrome model group.Metabolomics analysis revealed that arginine biosynthesis and purine metabolism pathways were markedly disrupted in the cold-syndrome model group,and compared with the cold syndrome model group,the contents of L-citrulline were significantly increased,whereas the contents of uric acid and xanthine were significantly decreased in the high-dose cold syndrome group(all P<0.05).Immunohistochemistry and enzyme-linked immunosorbent assay results indicated that,compared with the cold syndrome model group,TNF-α and IL-17 levels in synovial tissue and serum were significantly decreased(all P<0.05),whereas IL-10 levels were significantly increased(all P<0.01)in the high-dose cold syndrome group.Conclusion:Guanjie Zhentong Babu plaster alleviates joint injury in CIA rats with cold syndrome by regulating arginine and purine metabolism-related pathways to restore inflammatory balance.
