类风湿关节炎合并肺间质病变流行病学及危险因素研究现状OA
Rheumatoidarthritis-associated interstitial lung disease:Current research landscape on epidemiology and risk factors
类风湿关节炎合并肺间质病变(RA-ILD)是类风湿关节炎(RA)常见且严重增加患者死亡率的关节外并发症.近年来,随着高分辨率CT的应用,亚临床型患者的检出率不断上升,且全球流行病学呈现显著的地域异质性.RA-ILD的发病机制复杂,早期临床表现隐匿且病情进展迅速,精准识别相关风险因素是改善患者预后的关键环节.本文从遗传易感性、社会人口学与生活方式、临床特征、血清学标志物及临床用药等多个方面,系统综述了RA-ILD的流行病学特征及其发病与进展相关危险因素的最新研究进展,旨在为该疾病的早期筛查、风险分层及个体化治疗提供参考依据,以期推动临床管理向主动的"风险拦截"转变.
Rheumatoid arthritis-associated interstitial lung disease(RA-ILD)is a common and severe extra-articular complication of rheumatoid arthritis that significantly increases patient mortality.In recent years,with the application of high-resolution computed tomography,the detection rate of subclinical cases has been rising,and its global epidemiology demonstrates notable regional heterogeneity.The pathogenesis of RA-ILD is complex,characterized by subtle early clinical manifestations and rapid disease progression.Accurately identif-ying associated risk factors is a crucial step in improving patient prognosis.This article systematically reviews the latest research progress on the epidemiological characteristics of RA-ILD and the risk factors associated with its onset and progression,from the perspectives of genetic susceptibility,sociodemographics and lifestyle,clini-cal features,serological biomarkers,and clinical medications.This review aims to provide valuable insights for early screening,risk stratification,and personalized treatment of the disease,facilitating a paradigm shift in clinical management towards proactive"risk interception".
