便携式视频眼震视图仪设计与临床验证OA
Design and clinical validation of a portable video nystagmus visualization device
目的 验证便携式视频眼震视图仪在基层医疗机构眩晕筛查中的客观性和可重复性.方法 纳入2024年1月至2025年12月期间在苏州大学附属儿童医院耳鼻咽喉科门诊及苏州市吴中人民医院心内科就诊的患者20例(含儿童8例)设为眩晕组,其中男性9例,女性11例,平均年龄(46.35±10.24)岁.健康志愿者20名(含儿童5名)设为健康对照组,其中男性10名,女性10名,平均年龄(44.80±11.15)岁.两组间年龄、性别构成差异无统计学意义(t=1.57,P=0.123;χ²=2.88,P=0.087),具有可比性.在Dix-Hallpike试验和滚转试验中,同步采用便携式视频眼震视图仪、智能手机录像及医师肉眼观察3种方式记录患者眼震表现.所有采集视频均由两名高年资医师独立进行盲法判读,内容包括眼震是否出现、眼震优势方向以及眼震持续时间.结果 便携式视频眼震视图仪成功采集眩晕组和健康对照组的眼震视频,图像清晰、帧间连续性好,无明显运动伪影.设备成功记录了10例单纯后半规管、4例多管受累及4例单纯水平半规管良性阵发性位置性眩晕的典型眼震,并为2例疑似患者提供了客观依据.在眼震方向识别上,便携式视频眼震视图仪的判读结果与临床综合诊断的一致性高达95%(38/40),Kappa值为0.90,显示出极好的一致性;显著优于智能手机录像(一致性82.5%,Kappa=0.65)和医师肉眼观察(一致性77.5%,Kappa=0.55).结论 便携式视频眼震视图仪操作简便、成本低,可有效提升眼震观察的客观性与准确性,尤其适用于基层医疗和儿童患者群体的眩晕筛查.
Objective To verify the objectivity and reproducibility of a portable video nystagmus visualization device for dizziness screening in primary healthcare settings.Methods A total of 40 participants were enrolled from January 2024 to December 2025 at the Children's Hospital Affiliated to Soochow University and Suzhou Wuzhong People's Hospital,including a vertigo group(n=20 including 8 children,9 males and 11 females,mean age=46.35±10.24 years)and a healthy control group(n=20 including 5 children,10 males and 10 females,mean age=44.80±11.15 years).The two groups were matched in age and gender(t=1.57,P=0.123;χ²=2.88,P=0.087).Nystagmus was simultaneously recorded During the Dix-Hallpike and roll tests using the portable video nystagmus visualization device,smartphone video,and direct visual observation.All video recordings were independently reviewed by two blinded senior clinicians to assess the presence,dominant direction and duration of nystagmus.Results The portable device successfully captured nystagmus videos from both groups without significant motion artifacts,including nystagmus in BPPV involving the posterior canal in 10 cases,multi-canal involvement in 4 cases,and horizontal canal in 4 cases,and provided objective evidence of nystagmus in 2 suspected cases.The nystagmus direction identified by expert interpretation using the device agreed with clinical diagnosis in 38 cases(95%,Kappa=0.90),indicating excellent consistency.This was significantly superior to smartphone recording(82.5%,Kappa=0.65)and naked-eye observation(77.5%,Kappa=0.55).Conclusions The portable video nystagmus visualization device is user-friendly and cost-effective.It effectively improves the objectivity and accuracy of nystagmus observation and is particularly suitable for dizziness screening in primary care settings and in pediatric populations.
