基线非传统血脂与脑小血管病影像总负荷对突发性耳聋预后的预测价值OA
Values of baseline non-traditional lipid parameters and load of cerebral small vessel disease in predicting sudden sensorineural hearing loss outcomes
目的 探讨不同脑小血管病(cerebral small vessel disease,CSVD)影像总负荷与基线非传统血脂指标对突发性耳聋预后的预测价值.方法 纳入胜利油田中心医院2020年5月至2024年6月收治的363例突发性耳聋患者为研究对象,根据纯音平均听阈将患者分为轻度和中度听力损失组52例、中重度和重度听力损失组160例、极重度听力损失和全聋组151例.收集临床资料,分析其影像学特征,通过MRI评价CSVD影像负荷评分.比较不同组的临床特征及治疗效果.结果 3组高血压病、糖尿病、总胆固醇、甘油三酯、高密度脂蛋白胆固醇、低密度脂蛋白胆固醇、全聋型构成比比较,差异有统计学意义(P<0.05).CSVD影像负荷评分较高(1分、2分≥3分)及较高脂蛋白联合指数(lipoprotein combine index,LCI)、血浆致动脉粥样硬化指数(atherogenic index of plasma,AIP)是突发性耳聋预后不良的独立危险因素(P<0.05).CSVD影像负荷评分、卡斯特利风险指数I、卡斯特利风险指数Ⅱ、LCI、AIP预测突发性耳聋预后的受试者特征曲线下面积分别为0.697、0.560、0.560、0.594、0.613.AIP联合影像负荷预测突发性耳聋患者预后的曲线下面积为0.733,优于各指标单独预测.结论 CSVD影像总负荷评分和AIP、LCI等非传统血脂参数是评估突发性耳聋预后的有意义指标,且二者联合可显著提升预测效能.
Objective To determine values of baseline non-traditional lipid parameters and load of cerebral small vessel disease(CSVD)in the prognosis of sudden sensorineural hearing loss.Methods Hearing status in 363 patients with sudden deafness admitted to Shengli Oilfield Central Hospital from May 2020 to June 2024 were categorized as mild or moderate(n=52),moderate-severe or severe(n=160)and profound or total deafness(n=151)based on pure-tone average(PTA).Clinical manifestations and radiological features were analyzed,as well as MRI load of CSVD.Clinical characteristics and therapeutic outcomes were compared among patients with different levels of hearing loss.Results Prevalence of hypertension and diabetes,levels of total cholesterol(TC),triglyceride(TG),high-density lipoprotein cholesterol(HDL-C),low-density lipoprotein cholesterol(LDL-C),and ratios of total deafness,were significantly different for different the three hearing status groups(P<0.05).High MRI load of CSVD(1 points,2 points and≥3 points)and high LCI and atherogenic index of plasma(AIP)were independent risk factors for poor prognosis(P<0.05).The area under the curve(AUC)for predicting prognosis was 0.697 for MRI load of CSVD,0.560 for Castelli's risk index-I(CRI-I),0.560 for Castelli's risk index-Ⅱ(CRI-Ⅱ),0.594 for lipoprotein combine index(LCI)and 0.613 for AIP,respectively.The AUC of AIP and MRI load of CSVD combination for predicting prognosis was 0.733,significantly better than each indicator alone.Conclusions MRI load of CSVD and non-traditional blood lipid parameters such as AIP and LCI are useful indicators for assessing the prognosis of sudden deafness,and their combination can significantly enhance predictive efficacy.
