外耳道CT测量参数在耳科手术中的临床应用价值OA

Computed tomography measurement parameters of the external auditory canal:applications in otological surgery

目的 对比分析外耳道各解剖部位的CT参数,为耳内镜手术方案的规划提供精准依据.方法 回顾性分析2020年6月至2023年6月就诊于空军军医大学西京医院的43例(43耳)慢性化脓性中耳炎并行Ⅰ型鼓室成形术患者的临床资料,根据术中是否行环形外耳道成形术分为干预组和对照组.记录患者的年龄、性别、手术侧别、穿孔部位、穿孔大小及影像学测量数据,采用SPSS 26.0软件进行数据统计分析,绘制受试者工作特征曲线评估外耳道CT解剖测量参数在反映外耳道狭窄程度时的灵敏度和特异度.结果 两组患者在性别、年龄、手术侧别、鼓膜穿孔大小上并无显著差异,但在穿孔部位上有显著差异(P<0.05).干预组和对照组外耳道峡部的前后径及上下径、骨性外耳道口的前后径、耳道前壁及下壁弯曲角度在两组之间均具有显著差异(P<0.05).其中外耳道峡部的前后径和上下径反映外耳道狭窄的特异性最高.当外耳道峡部的前后径≤4.5 mm或外耳道峡部上下径≤7.0 mm时,提示外耳道骨质可能对鼓膜暴露存在遮挡而需行外耳道成形术.当外耳道下壁弯曲角度≤136.7°时,同样对手术方式会造成一定影响.结论 外耳道作为耳内镜手术的必经路径对手术操作具有直接影响,分析颞骨CT的外耳道峡部前后径、上下径和外耳道下壁弯曲角度,对术前精细评估外耳道条件具有重要价值.

Objective To comparatively analyze computed tomography(CT)parameters of key anatomical regions within the external auditory canal(EAC),and provide a precise basis for preoperative planning of transcanal endoscopic surgical procedures.Methods Data of 43 patients(43 ears)diagnosed with chronic suppurative otitis media who underwent typeⅠtympanoplasty at Xijing Hospital of the Air Force Medical University between June 2020 and June 2023 were reviewed.The patients were divided into an intervention group and a control group based on whether circular canaloplasty was performed during the surgery.Age,gender,surgical side,perforation location and size,and imaging measurements were collected.The SPSS 26.0 software was used for statistical analysis.Receiver operating characteristic(ROC)curves were constructed for sensitivity and specificity of external auditory canal measurements on CT scans in reflecting external auditory canal stenosis.Results There were no significant differences between the two groups in gender,age,surgical side and size of perforation,but there were significant differences in the location of perforation(P<0.05).Differences between the two groups were seen in the anterior-posterior and superior-inferior diameters of the EAC isthmus,anterior-posterior diameter of the canal orifice,and the curvature angles of the anterior and inferior canal wall overhangs(P<0.05).Among them,the anterior-posterior(when≤4.5 mm)and superior-inferior(when≤7.0 mm)diameters of the isthmus showed high specificity in reflecting stenosis that might hinder visualization of the tympanic membrane and require canaloplasty.Additionally,a curvature angle of the inferior canal wall overhang≤136.7° could also influence the choice of surgical approach.Conclusions As the mandatory access route for transcanal endoscopic surgery,the external auditory canal has a direct impact on the surgical procedure.A detailed measurement of the anterior-posterior and superior-inferior diameters of the EAC isthmus and the curvature angle of the inferior wall on temporal bone CT is of significant value for meticulous preoperative evaluation.
