基于"玄府气液"理论辨治眼底血症OA

Xuanfu is a fundamental concept in Traditional Chinese Medicine(TCM)theory describing the microscopic structural pathways in the body,responsible for the movement of Qi and body fluids(Qiye)as well as information transmission.This article proposes that the core pathogenesis evolution of ocular fundus hemorrhage follows a dynamic chain:Organ dysfunction-Xuanfu disorder-Qiye dysregulation-collateral damage-blood extravasation-Xuanfu obstruction-vision impairment.The central mechanism involves dysfunction of organs leading to stagnation in ocular Xuanfu,abnormal Qiye movement resulting in collateral damage and bleeding.Extravasated blood,as a pathological product,further obstructs Xuanfu,creating a vicious cycle and ultimately causing severe visual impairment.The clinical treatment focuses on unblocking Xuanfu,using Mingyi Tongxuan Decoction as a base prescription.This formula integrates pungent aromatics to open Xuanfu,tonify Qi to control bleeding,and activate collaterals to remove stasis.It aims to restore the normal flow of Qiye,harmonize Qi and blood,and promote vision.This approach provides a new theoretical perspective and diagnostic framework for understanding the nature of ocular fundus hemorrhage in TCM and improving clinical efficacy.
