目的 观察磁共振扩散张量成像(DTI)对甲状腺相关眼病(TED)患者的视神经定量分析中的应用价值.方法 纳入2023年11月—2024年9月就诊于中国中医科学院眼科医院的TED患者21例(42只眼),设定为TED组,另外纳入健康志愿者29例(58只眼)作为对照组;2组均进行眼眶DTI扫描,测量并比较2组视神经的部分各向异性指数(FA)、表观弥散系数(ADC)、平均弥散率(MD)、径向各向异性(RA)及视神经直径,并将测量值与TED进行相关性分析.结果 (1)视神经DTI图像表现:T1加权成像轴位(T1WI)图像中,TED患者一侧视神经边界欠清晰,周围眼外肌呈明显梭形增粗,软组织肿胀对视神经造成压迫;健康志愿者视神经走行自然,边界锐利.(2)视神经DTI测量值比较:TED组的视神经ADC值、MD值均高于对照组,视神经直径低于对照组,差异有统计学意义(tADC=4.749,P=0.029;tMD=43.173、t视神经=22.758,均P=0.000).2组FA值和RA值比较,差异均无统计学意义(均P>0.05).(3)视神经DTI测量值与TED的相关性:双侧视神经ADC值、MD值与TED呈正相关(rADC=0.218,P=0.029;rMD=0.553,P=0.000);双侧视神经的轴位值与TED呈负相关(r=-0.434,P=0.000).双侧视神经MD值与性别呈正相关(r=0.209,P=0.037),双侧视神经的轴位值与性别呈负相关(r=-0.268,P=0.007),均有统计学意义.结论 通过DTI技术测定TED患者相关参数的改变,可为定量评估其对视神经损伤的影响提供重要参考依据.

OBJECTIVE To observe the quantitative analysis of the optic nerve in patients with thyroid eye disease(TED)using magnetic resonance diffusion tensor imaging(DTI).METHODS A total of 21 patients(42 eyes)with TED who were treated at Eye Hospital,China Academy of Chinese Medical Sciences from November 2023 to September 2024 were enrolled as the TED group,and 29 healthy volunteers(58 eyes)were enrolled as the control group.Orbital DTI scanning was performed in both groups.Fractional anisotropy(FA),apparent diffusion coefficient(ADC),mean diffusivity(MD),radial anisotropy(RA),and optic nerve diameter were measured and compared between the two groups.Correlations between these measurements and TED were analyzed.RESULTS(1)Optic nerve DTI imaging findings:On axial T1-weighted imaging(T1WI),the unilateral optic nerve of TED patients showed ill-defined boundary,the surrounding extraocular muscles were obviously fusiform thickened,and soft tissue swelling caused compression on the optic nerve;The optic nerve of healthy volunteers presented natural course and sharp boundary.(2)Comparison of DTI parameters:ADC and MD values of the optic nerve were significantly higher,and optic nerve diameter was significantly lower in the TED group than in the control group(tADC=4.749,P=0.029;tMD=43.173,toptic nerve=22.758,both P=0.000).There were no significant differences in FA or RA values between the two groups(all P>0.05).(3)Correlation between optic nerve DTI measurements and TED:Bilateral optic nerve ADC and MD values were positively correlated with TED(rADC=0.218,P=0.029;rMD=0.553,P=0.000);axial values of bilateral optic nerves were negatively correlated with TED(r=-0.434,P=0.000).Bilateral optic nerve MD values were positively correlated with gender(r=0.209,P=0.037),while axial values of bilateral optic nerves were negatively correlated with gender(r=-0.268,P=0.007).All of the above differences were statistically significant.CONCLUSIONS Measurement of DTI parameters can provide important reference for quantitative evaluation of optic nerve injury in patients with TED.
