超声监测视神经鞘减压术后视神经鞘直径变化与视野恢复的相关性研究OA
Correlation between the ultrasound-monitored ONSD changes and the recovery of visual field after ONSF
目的:探讨视神经鞘减压术(ONSF)术后视神经鞘直径(ONSD)变化与视野恢复的时序性关联,为临床提供可量化的生物标志物.方法:选取2022年至2024年宣武医院收治的86例接受ONSF的患者,超声测量视神经球后3 mm矢状位ONSD、依据国际标准化视乳头水肿分级量表(Frisén)分级评估视乳头水肿,比较手术前后视野平均缺损(MD)值改变量(ΔMD).测量时间点为术前(T0),术后1周(T1)、1个月(T2)和3个月(T3).分析手术前后基线差异、重复测量单因素方差(ANOVA)评估时间效应,以及Pearson相关系数(r)考察各指标相关性.结果:患者术后ONSD持续下降,与术前比较差异有统计学意义(F=265.730,P<0.05).Frisén分级随时间(术后1周、1个月和3个月)改善,改善率达70.09%.相关性分析显示,术前,术后1周、1个月及3个月的不同时间点视野恢复各变量间均存在显著相关,并在手术前后呈现明显变化.ONSD与Frisén分级在术前,术后1周、1个月及3个月的不同时间点均呈显著正相关(r=0.843、0.782、0.711、0.623,P<0.05);ONSD与ΔMD值在术前呈显著负相关(r=-0.763,P<0.05),而术后1周、1个月及3个月的不同时间点均为显著正相关(r=0.695、0.772、0.824,P<0.05);Frisén分级与ΔMD值在术前呈显著负相关(r=-0.684,P<0.05),而术后1周、1个月及3个月的不同时间点为显著正相关(r=0.732、0.753、0.795,P<0.05).结论:超声标准化监测可动态量化ONSF术后ONSD,其与视野恢复呈显著时序性正关联,为疗效评价提供可靠的无创生物标志物.临床中可依此制定个体化随访策略.
Objective:To investigate the association with temporal sequence between the changes of optic nerve sheath diameter(ONSD)and the recovery of visual field after optic nerve sheath fenestration(ONSF),so as to provide a quantifiable biomarker for clinical practice.Methods:Eighty-six patients who underwent ONSF at Xuanwu Hospital between 2022 and 2024 were selected.The ONSD of sagittal position at 3mm behind eyeball was measured by ultrasound,and papilledema was assessed according to the internationally standardized Frisén scale for papilledema grading(Frisén).The variation of mean damage(MD)value(ΔMD)of visual field was compared.The time points of measures included before surgery(T0),at the 1st week after surgery(T1),at the 1st month after surgery(T2)and at the 3rd month after surgery(T3).The difference of baseline was analyzed,and the time effects were assessed by using repeated measures one-way analysis of variance(ANOVA).Pearson correlation coefficients(r)were used to analyze the correlation of various indicators.Results:The postoperative ONSD of patients continuously decreased,and the difference of that between before and after surgery was significant(F=265.730,P<0.001).With the change of time,Frisnuous grading improved,and the improvement rate reached to 70.09%.The correlation analysis indicated that there were significant correlations among different variables at before surgery,at the 1st week,1st month and 3rd month after surgery,there was significant change between before and after surgery.There were significantly positive correlations between ONSD and Frisérisnts grading at different time points included before surgery,at the 1st week,1st month and 3rd month after surgery(r=0.843,0.782,0.711,0.623,P<0.05).ONSD appeared significantly negative correlation with ΔMD values before surgery(r=-0.763,P<0.05),but it appeared significantly positive correlation with that at the 1st week,1st month and 3rd month after surgery(r=0.695,0.772,0.824,P<0.05).In addition,the Frisérisnts grading appeared significantly negative correlation with ΔMD values before surgery(r=-0.684,P<0.05),but it appeared significantly positive correlation with that at the 1st week,1st month and 3rd month after surgery(r=0.732,0.753,0.795,P<0.05).Conclusion:Standardized ultrasonographic monitoring can dynamically quantify the reduction of ONSD after ONSF,which appears significantly positive association with temporal sequence to visual field.It can provide reliable non-invasive biomarker for evaluation of therapeutic efficacy.The individualized follow-up strategy can be formulated according to this research in clinical practice.
