膈肌超声评估机械通气患者撤机的临床应用与研究进展OA
Clinical application and research progress of diaphragmatic ultrasound in assessing weaning from mechanical ventilation
机械通气是临床治疗呼吸衰竭的常见呼吸支持方式,主要是为改善机体的氧供需求,降低二氧化碳潴留,将萎陷肺泡重新展开以保证肺的通气换气功能恢复.在ICU中,大部分患者需要超过24 h的机械通气支持.但机械通气属于有创性操作,从开始使用的那一刻起,临床医生在考虑撤机的过程中必须考虑两个困难:一是不必要地延长机械通气时间,二是过早撤机,这两者均可增加发病与死亡风险.因此减少患者对呼吸机的依赖,对膈肌功能进行精准评估,有助于提高撤机成功率,改善患者的远期预后.为了确定安全撤机的最佳时间,现有的传统撤机指标均表现出不足的灵敏度和特异度,需要其他指标来提高临床决策的稳健性.膈肌作为人体主要的呼吸肌,可运用安全且便携式的超声技术,使其在机械通气患者的某一时间点上对膈肌厚度变化和位移分析来快速评估和可视化呼吸肌的功能状态,以实现良好的撤机目标.
Mechanical ventilation is a common respiratory support modality in the clinical management of respiratory failure,primarily aimed at improving oxygen supply and demand,reducing carbon dioxide retention,and reopening collapsed alveoli to restore lung ventilation and gas exchange function.In the ICU,most patients require more than 24 hours of mechanical ventilation support.However,mechanical ventilation is an invasive procedure,and from the moment it is initiated,clinicians must consider two challenges in the weaning process:one is unnecessarily prolonging mechanical ventilation duration,and the other is premature weaning.Both are associated with increased morbidity and mortality risks.Therefore,reducing patient dependence on the ventilator and performing precise assessments of diaphragmatic function can help increase the success rate of weaning and improve long-term patient outcomes.Traditional weaning indicators have shown insufficient sensitivity and specificity,highlighting the need for additional metrics to enhance the robustness of clinical decision-making.As the primary respiratory muscle,the diaphragm can be assessed using safe and portable ultrasound technology,allowing for the rapid evaluation and visualization of diaphragmatic function through thickness changes and displacement analysis at specific time points during mechanical ventilation.This approach helps achieve effective weaning outcomes.
