彩色多普勒超声参数、血清PANDER水平与GDM患者凝血功能、炎症反应的关系及临床意义OA

[Objective]To explore the relationship between color Doppler ultrasound parameters,serum pancreatic-derived factor(PANDER)levels,and coagulation function,inflammatory response in patients with gestational diabetes mellitus(GDM)and its clinical significance[Methods]A retrospective study was conducted on 60 patients with diabetes during pregnancy who visited Huiji District People's Hospital of Zhengzhou from February 2023 to February 2024,and these patients served as the study group.Additionally,60 healthy pregnant women who underwent physical examinations during the same period were selected as the control group using a 1:1 matching principle.The color Doppler ultrasound parameters of uterine artery blood flow[systolic peak flow velocity and diastolic end flow velocity ratio(S/D),pulsatility index(PI),resistance index(RI)]and serum PANDER levels,as well as coagulation function indicators[prothrombin time(PT),activated partial thromboplastin time(APTT),fibrinogen(FIB)]and inflammatory markers[fatty acid-binding protein 4(FABP4),interleukin-6(IL-6)]were compared between the two groups.The correlation between color Doppler ultrasound parameters,serum PANDER levels,and coagulation function and inflammatory response in patients with diabetes during the second trimester of pregnancy was analyzed.All patients in the research group were followed up until delivery.The color Doppler ultrasound parameters and serum PANDER levels were compared among patients with different pregnancy outcomes,and their evaluation value for pregnancy outcomes was analyzed.[Results]In the study group,PT and APTT were lower than those in the control group,while FIB,IL-6,and FABP4 were higher than those in the control group(P<0.05).Pearson correlation analysis showed that RI,PI,S/D,and serum PANDER levels were negatively correlated with PT and APTT,and positively correlated with FIB,IL-6,and FABP4 at admission(P<0.05).Compared with patients with good pregnancy outcomes,patients with poor pregnancy outcomes had higher RI,PI,S/D and serum PANDER levels at admission(P<0.05).The area under the receiver operating characteristic curves(AUCs)of RI,PI,S/D and serum PANDER for predicting adverse pregnancy outcomes in GDM patients were 0.755,0.734,0.768,and 0.741,respectively.The combined prediction AUC of multiple indicators was 0.923,with a sensitivity of 85.71％and a specificity of 92.31％,which was superior to that of single-indicator prediction.[Conclusion]The uterine artery blood flow parameters obtained through color Doppler ultrasound and serum PANDER levels are closely related to the coagulation function and inflammatory response in patients with GDM.Clinically,they can be used to early assess the condition of GDM and predict pregnancy outcomes,thereby facilitating the formulation of targeted intervention plans.
