目的 探讨结直肠癌(CRC)组织中细胞外基质蛋白1(ECM1)、四连接同源基因(FJX1)表达水平,并分析其对术后5年预后的预测价值.方法 前瞻性选取2018年1月至2020年1月于濮阳油田总医院诊治的92例CRC患者为研究对象,比较癌组织、癌旁组织中ECM1、FJX1 mRNA及蛋白表达水平.分析不同临床特征患者癌组织中ECM1、FJX1蛋白表达水平.依据术后5年预后情况将CRC患者分为预后不良亚组、预后良好亚组,比较其癌组织中ECM1、FJX1 mRNA及蛋白表达水平.分析CRC患者术后5年预后中ECM1、FJX1的交互效应.评价ECM1、FJX1对术后5年预后的预测价值.结果 癌组织中ECM1、FJX1 mRNA及蛋白表达水平高于癌旁组织(P<0.05);预后不良亚组ECM1、FJX1 mRNA及蛋白表达水平高于预后良好亚组(P<0.05);根据患者ECM1、FJX1蛋白表达水平均值分为高水平、低水平,据此可获得4个亚组,分别为ECM1低水平+FJX1低水平(R00)、ECM1低水平+FJX1高水平(R01)、ECM1高水平+FJX1低水平(R10)、ECM1高水平+FJX1高水平(R11),以R00组为参照,R01、R10、R11可使术后5年预后不良的风险增加2.014、3.290、6.327倍(P<0.05);ECM1、FJX1蛋白表达水平对术后5年预后不良的影响存在正向相加交互效应,且AP=0.3197,由两者交互效应所致术后预后不良的占比为31.97%;ECM1、FJX1蛋白表达水平联合预测术后5年预后的曲线下面积(AUC)高于单项指标预测(P<0.05).结论 CRC癌组织中ECM1、FJX1表达水平较癌旁组织明显升高,术后5年预后不良患者ECM1、FJX1表达水平较预后良好患者明显升高,ECM1、FJX1蛋白表达水平对术后预后不良的影响存在正向相加交互效应,检测其水平对术后5年预后不良具有一定预测价值,联合检测其水平的预测价值更高.

[Objective]To investigate the expression levels of extracellular matrix protein 1(ECM1)and four-jointed box kinase 1(FJX1)in colorectal cancer(CRC)tissues,and to analyze their predictive value for 5-year prognosis after surgery.[Methods]A total of 92 CRC patients who were diagnosed and treated in Puyang Oilfield General Hospital from January 2018 to January 2020 were prospectively selected as the research objects.The mRNA and protein expression levels of ECM1 and FJX1 in cancer tissues and adjacent non-cancerous tissues were compared.The expression levels of ECM1 and FJX1 proteins in cancer tissues of patients with different clinical characteristics were analyzed.According to the prognosis of 5 years after operation,CRC patients were divided into poor prognosis subgroup and good prognosis subgroup,and the mRNA and protein expression levels of ECM1 and FJX1 in cancer tissues were compared.The interaction effect of ECM1 and FJX1 in the 5-year prognosis of CRC patients was analyzed.The predictive value of ECM1 and FJX1 for 5-year prognosis after surgery was evaluated.[Results]The mRNA and protein expression levels of ECM1 and FJX1 in cancer tissues were higher than those in adjacent tissues(P<0.05).The mRNA and protein expression levels of ECM1 and FJX1 in the poor prognosis subgroup were higher than those in the good prognosis subgroup(P<0.05).According to the average level of ECM1 and FJX1 protein expression in patients,they were divided into high level and low level,and four subgroups were obtained,which were low level of ECM1+low level of FJX1(R00),low level of ECM1+high level of FJX1(R01),high level of ECM1+low level of FJX1(R10),high level of ECM1+high level of FJX1(R11).R00 group was used as reference,R01,R10 and R11 could increase the risk of poor prognosis in 5 years after operation by 2.014,3.290 and 6.327 times(P<0.05).The expression levels of ECM1 and FJX1 proteins had a positive additive interaction effect on 5-year postoperative poor prognosis,and AP=0.3197.The proportion of postoperative poor prognosis caused by the interaction between the two was 31.97%.The area under the receiver operating characteristic curve(AUC)value of combined prediction of ECM1 and FJX1 protein expression levels for 5-year postoperative prognosis was higher than that of single index prediction(P<0.05).[Conclusion]The expression levels of ECM1 and FJX1 in CRC cancer tissues are significantly higher than those in adjacent non-cancerous tissues.In patients with poor 5-year postoperative prognosis,the expression levels of ECM1 and FJX1 are notably elevated than those with good prognosis.The expression levels of ECM1 and FJX1 proteins exhibit a positive additive interaction effect on postoperative poor prognosis.Measuring their levels holds predictive value for poor 5-year postoperative prognosis,and combining their levels for detection offers even higher predictive value.
