目的 筛选和鉴定食管鳞状细胞癌(ESCC)的性别相关预后基因,建立风险评分模型和预后列线图,利用临床样本验证关键基因的生物学特性,为临床提供有效的预后评估工具.方法 基于GSE53625数据集,采用加权基因共表达网络分析(WGCNA)筛选性别相关模块基因,与Kaplan-Meier(K-M)分析获得的预后基因取交集,最小绝对收缩与选择算子(LASSO)和多因素Cox回归分析进一步筛选性别相关预后基因,建立风险评分模型.结合风险评分与独立预后因素开发预后列线图,校准曲线、受试者操作特征(ROC)曲线和决策曲线分析(DCA)评估模型的临床应用效能.结果 根据风险评分模型的最佳截断值,患者被划分为高风险组和低风险组,两组总生存期(OS)存在显著差异.基于多因素Cox回归分析结果构建的预后预测模型涵盖患者年龄、治疗方式和风险评分.与TNM分期相比,模型显示出良好的校准和鉴别能力.进一步分析表明,IL5RA高表达的患者预后较好,女性预后优于男性.结论 本研究鉴定出的性别相关预后基因有望成为新的预后标志物,为ESCC的精准治疗和预后评估提供重要参考.

[Objective]To identify gender-related prognostic genes in esophageal squamous cell carcinoma(ESCC)through bioinformatics analysis,develop a risk scoring model and prognostic nomogram,and analyze the biological characteristics of key genes in clinical samples,thereby providing an effective tool for prognostic assessment in clinical practice.[Methods]Using the GSE53625 dataset,weighted gene co-expression network analysis(WGCNA)was used to screen gender-related module genes,which were intersected with prognostic genes obtained from Kaplan-Meier(K-M)analysis.Gender-related prognostic genes were further identified using least absolute shrinkage and selection operator(LASSO)and multivariable Cox regression analysis,and a risk score model was constructed.A prognostic prediction nomogram was developed by integrating the risk score and independent prognostic factors.The clinical application performance of the model was evaluated using calibration curves,receiver operating characteristic(ROC)analysis,and decision curve analysis(DCA).[Results]Based on the optimal cutoff value of the risk score model,patients were stratified into high-risk and low-risk groups,with significantly different overall survival(OS)between the two groups.The constructed prognostic nomogram,which incorporated patient age,treatment modality,and risk score,demonstrated excellent calibration and discrimination capabilities.Further analysis revealed that patients with high IL5RA expression had relatively better OS,and female patients exhibited superior survival outcomes compared with males.[Conclusion]The identified gender-related prognostic genes hold promise as novel prognostic biomarkers,providing valuable insights for precise treatment and prognosis assessment in ESCC.
