药物转运体介导的药物-药物相互作用研究进展OA

Advances in research on drug-drug interactions mediated by drug transporters:a review

药物转运体是调控众多药物体内过程的关键膜蛋白,其介导的药物-药物相互作用(DDIs)直接影响药物药代动力学与临床用药安全,是药物研发和临床用药的核心评估内容.全球主流药品监管机构均出台专项指南,明确核心转运体DDIs风险评估规范.本文系统梳理转运体介导DDIs研究现状,重点解读美国食品药品监督管理局(FDA)、欧洲药品管理局(EMA)和日本药品医疗器械综合机构(PMDA)三大主流监管机构的转运体DDIs指导原则的差异及核心要求,阐述体外研究模型与体内评价策略的应用进展,聚焦体外-体内外推(IVIVE)核心难题,总结相关风险防控策略.目前该领域仍存在个体差异机制不明确、IVIVE精度不足、特殊人群数据匮乏等问题,未来需优化研究模型与预测方法,深化代谢-转运协同作用研究,为药物研发风险管控和临床精准用药提供理论支撑.

Drug transporters are key membrane proteins that regulate the disposition of numerous drugs within the body,and transporter-mediated drug-drug interactions(DDIs)directly impact drug pharmacokinetics and clinical medication safety,representing a core component of evaluation in both drug development and clinical practice.Major global regulatory agencies have issued specific guide-lines to standardize the evaluation of DDIs risks involving key transporters.This review summarizes the current research landscape of transporter-mediated DDIs,with a focus on interpreting the differences in core requirements and regulatory expectations outlined in the guidelines of the three major regulatory authorities—the Food and Drug Administration(FDA),European Medicines Agency(EMA)and Phar-maceuticals and Medical Devices Agency(PMDA).The advances in the application of in vitro-in vivo extrapolation(IVIVE)strategies are outlined,major challenges to IVIVE are underscored,and risk man-agement strategies are recommended.Current challenges in this field include a poor understanding of inter-individual variability,inaccuracy in IVIVE,and a lack of data from special populations.Future research should focus on optimizing experimental models and predictive methodologies and advancing the understanding of metabolic-transport interplay in order to provide evidence for risk management in drug development and precision medication.
