基于修正后模糊冲突模型的我国门诊共济政策执行困境研究OA
The execution dilemma of the outpatient mutual-aid policy in China based on the modified fuzzy conflict model
目的 剖析我国"门诊共济"政策执行制约因素,为优化政策实施效能提供理论依据.方法 依托修正后的"模糊-冲突"理论框架,构建分析矩阵,对2021年医改以来市级以上"门诊共济"政策文本与反馈数据进行文本分析.结果 政策传导阶段:模糊性递减,冲突性递增,执行呈政治性;政策深化阶段:二者同步减弱,模式转为行政性执行.结论 当前政策为行政执行模式,其核心制约要素为资源保障水平.执行过程面临政策模糊性与制度碎片化叠加、资源配置结构性失衡加剧、政策合法性基础与实施效能双重考验、行政驱动力与社会诉求存在适应性落差等多重困境.建议从构建分层协同的政策适配体系,创新财政资源分配与补偿机制,建立多维度政策效能评估系统,打造政社协同治理生态圈等方面确保"门诊共济"政策有效实施.
Objective To analyze the constraining factors in the implementation of the outpatient mutual-aid policy in China,aiming to provide a theoretical foundation for optimizing the effectiveness of policy im-plementation.Methods Based on the revised"ambiguity-conflict"theoretical framework,an analytical matrix was constructed for the textual analysis of documents of the outpatient mutual-aid policy and feedback data at the municipal level and above since the 2021 healthcare reform.Results The policy conveyance stage was character-ized by decreasing ambiguity and increasing conflict,prompting political implementation;the policy deepening stage saw a simultaneous reduction in both,shifting the model to administrative execution.Conclusions The cur-rent policy follows an administrative execution model,with resource assurance as its main constraint.The imple-mentation faces challenges such as policy ambiguity,institutional fragmentation,worsening imbalance in resource allocation,tests of legitimacy and effectiveness,and gaps between administrative actions and social demands.To ensure the effective implementation of the outpatient mutual-aid policy,it is suggested to build a collaborative poli-cy adaptation system,innovate financial resource allocation and compensation mechanisms,establish a multi-di-mensional effectiveness assessment system,and create a governance ecosystem that fosters collaboration between government and society.
