基于处方前置审核系统的G6PD缺乏症患者用药安全审核体系的构建与应用OA
Development and implementation of an audit system based on the pre-prescription review system for medication safety in patients with glucose-6-phosphate dehydrogenase deficiency
目的 基于处方前置审核系统体系构建葡萄糖-6-磷酸脱氢酶(G6PD)缺乏症患者用药安全审核体系,以规范医师处方行为,保障患者用药安全.方法 整合G6PD缺乏症的禁忌药物数据库(含中西药),将其嵌入美康合理用药系统的处方前置审核模块.依据药物禁用/慎用等级设置分级拦截规则,并通过三级审核机制实现处方实时拦截.结果 该系统运行5个月,共拦截含禁忌药物的不合理用药问题63条(涉及19种药品),系统触发拦截率100%,医师后续调整用药方案的比例达55.56%.系统拦截耗时1~2 s,药师在线审核平均耗时13.01 s,与用药安全审核体系应用前相比,审核效率得到了显著提高.结论 基于处方前置审核系统的专病化用药安全审核体系能使G6PD缺乏症患者用药错误的发生数量显著减少,通过分级拦截机制平衡安全性与诊疗灵活性,弥补传统人工审方的不足.该模式为医院构建低成本、高效率的用药安全体系提供了实践范例,有助于提高药师审方效率与临床合理用药水平.
AIM To construct an audit system based on the pre-prescription review system for medication safety in patients with glucose-6-phosphate dehydrogenase(G6PD)deficiency,aiming to standardize physician prescribing practices and ensure patient medication safety.METHODS A G6PD deficiency contraindicated medication database(including Chinese and Western medicines)was integrated and embedded into the pre-prescription review module of the Medicom PASS system.Graded alert rules were established based on drug contraindication levels(contraindicated/use with caution),and real-time prescription interception was implemented via a three-tier review mechanism.RESULTS Over 5 months of operation,the system intercepted 63 inappropriate prescriptions containing contraindicated medications(involving 19 distinct drugs),achieving a 100%alert trigger rate.Physicians subsequently adjusted medication regimens in 55.56%of these cases.The system performed each interception within 1 to 2 seconds,while the average time required for online pharmacist review was 13.01 seconds per order.Compared to the period before the implementation of the audit system based on the pre-prescription review system for medication safety,prescription review efficiency demonstrated a significant improvement.CONCLUSION Based on the pre-prescription review system,the disease-specific medication safety review framework significantly reduces medication errors in patients with G6PD deficiency.Its graded alert mechanism effectively balances medication safety with clinical flexibility,overcoming the limitations of traditional manual review.This model provides an empirical reference for hospitals to establish cost-effective and efficient medication safety systems,contributing to the optimization of pharmacist review efficiency and clinical rational medication use levels.
